Contract truck driver was loaded at the Agg Plant and while descending the hill to the scales, lost EE's brakes, jumped out of the dump truck receiving facial injuries and the truck continued down the road driverless through two berms before finally coming to rest a third berm.

EE was operating a CAT 773 haul truck when EE hit the left side of head on the inside of the operator's cab after hitting a pothole/bump in the road. After seeking medical attention, EE was given medications in the ER but did not receive a written prescription and was released to full duty.

Hauler operator was driving back to the strip after dumping EE's load at the waste area. There was a washout on the road and as EE tried to avoid some of the washout EE's right front tire hit a rock and the cab of the articulated truck EE was driving shifted. EE said EE felt a pain in EE's left lower back down into EE's leg. The employee was taken to be medically examined.

Tramming skid steer to assist in cleanup of debris around a work site. While tramming, employee ran over a rock that caused the machine to raise up on its back tires and then slam back down on the front. This caused the machine to bounce and injured the operator's neck.

After completing repair of the electronic eye that monitors belt rotation of the #10 belt, employee was returning to the man lift. While fully protected from falling by a fall protection system employee was preparing to re-enter the man lift. As man lift boomed closer employees fingers (left ring and pinky) were caught between the man lift and the structure.

Employee had been transporting water to fill the pond used by the wash plant. On EE's second trip as EE attempted to spray the water from the front sprayers into the pond, the truck moved forward over the berm into the pond. The employee exited the truck through the side door and made EE's way to the water's edge. EE sustained a cut on EE's right ear.

Employee was operating the Cat 980G loader, working the 3' stock pile and dressing up the berms. Employee claims the bucket was loaded and in the raised position with the loader articulated slightly to the left when the bucket suddenly free fell to the ground. This action caused the cab to bounce forward and the rear of the loader to rise from the ground then settle back.

The person injured was operating the water truck and while driving down the road the truck shifted gears as the operator hit a pothole in the road. This is when the operator reported that he hurt his back. The operator went to the hospital and missed the next two days of work before returning on the 3rd day.

Operator was prying on a large rock in the pit, he alleged his bucket slipped off and jarred the machine throwing him forward. Made a comment his back wasn't feeling great to the leadman but continued working until 7/15 when the doctor told him to take time off.

Injured employee was operating a front end loader on the load out yard. He hit a pot-hole and immediately felt pain in his upper back. He is out of work waiting for a MRI.

Employee was adjusting skirt boards on a chute which leads to a 42 belt. The belt was running empty to calibrate the scales. The employee finished adjusting the skirt boards when his hammer and left arm were sucked into a belt idler. The employee received minor burns on his arm, minor cuts on his wrist, and a laceration on his middle finger which required 8 stitches.

Employee drove a Kamatsu WA600 Wheel Loader into the muck pile to transport raw material to the primary crusher. As the loader entered the muck pile, the loader hit something making the machine come to a violent stop, straining the operators lower back. It is still unknown what the bucket hit to make the machine come to a sudden stop.

Employee was stripping in the pit, against the supervisor orders. Instead of moving the stripping piles he was digging into the wall repeatedly, a rock fell hitting the loader bucket. Damages to the loader are >$20000. He told management by phone he was having neck pain, going to the doctor. Refused to take a drug test. 7/11/2005: turned in his handbook and uniforms.

employee was using a bucket truck to access BC-2. The bucket cable broke, bucket fell to the bed of the truck. Employee continued to work, has since seen physician, monitored by physician and now needs surgery on 1/23/2006. This incident has just become reportable, which explains the gap in time between occurance and reporting. Has not missed any time.
