Was putting a conveyor chain in and it got stuck. When EE got it unstuck it pulled EE's left hand into the conveyor cutting the tip of EE's index finger.

Showing all 9EE was dragging vac-truck hose, when the hose caught on something, and strained elbow. Physician prescribed restricted duty until MRI determines damage.
Employee was assisting the emptying of dust collector hopper & unchoking tipping valve when he reached inside of the tipping valve to remove a piece of material. The screw conveyor below the tipping valve removed middle fingertip. Employee returned to work after 1 day away from work.
EE experienced discomfort in lower back while operating loader, but did not feel that medical evaluation was necessary, until approx. 4 months later when he asked to see a physican due to recurrence of discomfort. Doctor prescribed modified duty.
The employee sustained a strain when a large rock was dropped from a payloader into the bed of his dump truck. This took place on 6/20/03 but the injury did not become reportable until 5/24/05 when the employee missed work due to the doctor taking him off work.
Two employees were working on a reducer. It would not move so as one employee went to jog the belt, the other for some reason had his hand on the belt. The belt started and cut the end of his fingers.

WHILE RE-BRICKING THE INSIDE OF THE KILN, A FORKLIFT TRANSPORTING A PALLET OF BRICK HIT A ROUGH SPOT & BRICK FELL OFF & HIT A NEARBY EMPLOYEE.
