THE OPERATORS WERE DOING THE INSPECTIONS AT THE BEGINNING OF EACH SHIFT AND ADDING T HE NECESSARY BRAKE FLUID. THIS OPERATOR DID NOT MAKE THE BEGINNING INSPECTION AND THE EQUIPMENT RAN OUT OF BRAKE FLUID CAUSING THE BRAKE FAILURE.
