An employee was attempting to clear a blockage in the dust collector chute. While reaching in to free a pole that was being used, the employee's glove became entangled in a rotating component of the equipment. This resulted in the amputation of three fingers.

Showing all 18While loading a truck, a rock fell from the loader bucket striking the handrail. The rock bounced off the handrail and went into the door window causing the glass to break.
Operator was utilizing a bob cat walk behind for clean up. Operator was dumping product of mud in the sump when the walk behind became unstable bucking the operator off the back of the machine landing on their back.
Employee hit a bump on forklift and jammed neck.
Employee complained of a sore back. First aid was administered employee had another incident with a reported fall from a grader, once again first aid administered. Later on the employee was prescribed medication that our system failed to catch when it went into a medical treatment case
Two employees were involved. One employee was driving forward in a forklift, second employee was placing straps on a bag for a rail car when a bag was moved forward by the forklift and caught injured (2nd) employee's ankle between two pallets.
Employee traveling to Wilmington, CA - while making a u-turn at intersection of 5th Street and Pine Street in Longbeach, CA, another vehicle on employee's right side collided on the front/right passenger side of her vehicle. Soreness to neck and left shoulder.
At approx 3:30 am an MT4400 empty haul truck was found rolled over onto passenger side of the vehicle at the dump. Employee sustained slight fracture to T-1, and some abrasions to arms/face. Employee was transported to Barstow Community Hospital, treated and released.
Employee was stowing personal effects into the trunk of his personal vehicle in the employee parking lot after he had completed his shift. Another employee (who had also just completed his shift) started his personal vehicle, and his foot slipped off the clutch and his truck lurched forward hitting the injured employee.
Employee alleges while he was driving his haul truck, he leaned forward to adjust his seat and the truck hit a pot hole causing truck to "buck" causing him to strike his mouth on the steering wheel chipping his front tooth.

WHEN THE OPERATOR LEANED OVER TO TURN AIR CONDITIONING UNIT, DUST BLEW OUT OF THE VENT INTO HIS FACE, GETTING DUST PARTICLES IN HIS LT. EYE. EE WAS WEARING SAFETY GLASSES AT THE TIME. EE WAS SENT TO E.R. WHERE METAL OBJECT WITH RUST RING WAS REMOVED FROM HIS LT. EYE AND OPTHALMIC MEDICATIONS WERE ORDERED.
WHILE DRIVING UNIT RIG 42 ON THE NORTH DUMP, EE HIS A POTHOLE WITH L FRONT TIRE, WRENCHING HIS BACK. EE NOTIFIED SUPERVISOR AT END OF SHIFT, BUT REFUSED TO BE SEEN AT THAT TIME STATING HE WANT ED TO GO HOME AND SEE IF HE FELT BETTER. UPON RETURNING TO WORK THE FOLLOWING NIGHT, EE REPORT TO WORK THE NEXT NIGHT WITH COMPLAINT OF CONTINUED PAIN IN BACK, WAS SENT IMMEDIATELY TO THE HOS
EE STATES HE WAS OPERATING A UNTI RIG HAUL TRUCK AND THE SEAT WAS ADJUSTED FOR SOMEONE LIGHTER. WHEN HE WAS TRAVELING OVER ROUGH ROAD AT LESS THAN 5 MPH, THE SEAT BOTTOMED OUT, JARRING HIS BAC K.
WHILE EMPLOYEE WAS STANDING NEXT TO THE FORKLIFTTALKING TO THE OPERATOR, THE OPERATOR TURNED THEFORKLIFT AND STARTED FORWARD. THE REAR WHEEL STARTED TO ROLL OVER EMPLOYEE'S FOOT. HE YELLED AND THE OPERATOR BACKED UP.
THE EE STATES HE WAS ATTEMPTING TO PLACE A RUBBER PATCH OVER A HOLE IN THE 10 MOL DRYER FEED SCREW HOUSING. HE USED HIS RIGHT HAND TO PUSH THE EXISTING PATCH OF THE HOLE. IN DOING SO, HIS GLOV E WAS PULLED INTO THE SCREW, AMPUTATING HIS THUMB TO THE FIRST KNUCKLE.
EMPLOYEE STATES HE WAS OPERATING A SKIP ON AN EXTREMELY BUMPY ROAD AND FELT A TWEAK IN THE BACK.
