Showing all 4A short service employee (SSE) reported being injured while operating a forklift as he performed his warehouse duties. The SSE reported that he drove over a small crack in the concrete pad. The employee reported that the resulting jolt caused him to experience pain to the neck and lower back.
Forklift op was backing out of the bagging room and struck on of the contract EE's that was standing near overhead door with the forklift. The EE fell down and the forklift ran over his leg. The EE was taken to local ER for treatment and released. X-rays were negative and he had no fractures. He received presciption medication for pain.
Employee went up on top of crusher to help unplug it. He put the basket up along side of handrail. When he got out of the basket it popped up and pinched his index finger between the handrail on crusher and basket of manlift.
