WHILE OPERATING THE TRAIN, THE EMPLOYEE THOUGHT THE LOAD FELT LIGHT, STOPPED, PUT ON THE PARK BRAKE, AND WAS GOING TO SEE WHAT THE ISSUE WAS. BEFORE EXITING THE TRAIN, TRAIN CARS THAT CAME OFF OF THE TRAIN HIT THE BACK CAUSING THE EMPLOYEE TO FALL FORWARD IN THE TRAIN AND HIT LEFT LEG AND LEFT CHEEKBONE.
EE WAS RIDING IN ROXOR, BRAKES WENT OUT, HIT PILLAR CAUSING EE TO HIT HEAD ON ROOF OF ROXOR. 8/8/24 Injury Happened- The EE sought medical attention - reported they were personal injuries. 8/16/24 - EE ask for medical leave for a personal injury from (8/15-9/8). 9/9/2024 - The EE reported original injury 8/8/24 as a work injury that resulted of an alleged accident at work.
EMPLOYEE REPORTED ISSUE ON 07/15/2024 BUT DID NOT GO TO THE DOCTOR. EMPLOYEE THEN CONTACTED REPRESENTATIVE AT DOE RUN ON 09/23/2024 ABOUT HAVING ISSUES. EMPLOYEE DID NOT GO ON LIGHT DUTY UNTIL AFTER EE'S SURGERY ON 06/04/2025. EMPLOYEE STATES EE WAS MUCKING AND HAULING OUT OF A ROUGH AREA IN THE MINE. AFTER THIS THE EMPLOYEE STARTED HAVING LOWER BACK ISSUES.
Employee was operating a stand up ditch witch skid steer. The stand-up ditch witch caught a hose on the wall in the tunnel on the back door cover plate on the hydraulic side, which pulled the machine sideways and propped the door like it was running over a rock, and when it did it caught the right thumb on the hand rest, there for partially amputating the right thumb
EMPLOYEE WAS DRIVING HAUL TRUCK, SEAT BOTTOMED OUT CAUSING LOWER BACK AND KNEE PAIN
WHEN TRYING TO BACK UP, TRACTOR WENT OVER THE DITCH, CAUSED THE TRACTOR TO LUNGE BACKWARDS AND PINNED EMPLOYEE BETWEEN THE SEAT OF TRACTOR AND A 10" DISCHARGE WATER LINE.

BRAKES WENT OUT ON LOADER, LOADER HIT PILLAR, EMPLOYEE HIT LEFT SHOULDER ON STEERING WHEEL AND RIGHT SIDE OF HEAD ON METAL BRACKET INSIDE CAB.
EE OPERATING SCALER DOWN ROADWAY, HIT POTHOLE IN ROADWAY, BOUNCED IN SCALER AND FELT PAIN IN LOW BACK. EE WENT FOR MEDICAL EVALUATION ON 01/03/2023 AND WAS PLACED ON RESTRICED DUTY AT THAT TIME. EE WAS RELEASED TO FULL DUTY ON 01/09/2023.
EE riding in buddy seat while task training another employee. EE operating the truck hit a bump in the road causing the EE to hit head on cab. EE placed on restricted duty following medical evaluation.
EE alleged on 9/26/22 a pain in arm after operating loader on 9/25/22. EE placed on restricted duty following medical evaluation.
EE was riding in a buddy seat of a haul truck and felt pain in the upper back. EE was taken for medical attention at that time. EE remains on restricted duty.
Apx. 760' HPDE pipe was being pulled down an access road. Around a 100 degree bend, the pipe developed resistance from unidentified debris (presumed rock). The resistance gave way and the tension applied from pulling the pipe caused it to straighten striking and employee inspecting for holes.
EE turning steering wheel and felt pain in right shoulder. EE placed on restricted duty following medical treatment.
EE leaned out the window of a truck to adjust the mirror and felt pain in right knee. EE received medical treatment and was placed on restricted duty at that time. Treatment on-going.
Fuel buggy lunged forward pinning employee against pillar. EE was taken by ambulance to University of Missouri a Columbia and remains hospitalized currently. Amputation of left leg below the knee. EE remains off work.

EE was driving a haul truck underground when his light belt hit the bed lever, the bed raised and hit the back suddenly stopping the truck. When the truck stopped suddenly, EE hit his mouth on the steering wheel causing a laceration. EE received stitches and was returned to work at full duty.

EE was assisting in installing an explosive box, as the forklift guided the box down EE got left middle finger pinched between box and truck. Laceration to left middle finger requiring stitches. Restricted duty until x-ray results are known.

EE was drove haul truck over a boulder in the road and injured his back, hip and shoulder. He was given a second injection in his back on August 11, 2011 making this a MSHA Reportable.
Employee was driving through a rough spot in the road and felt pain in back. After continual treatment, employee began losing time on 09/03/2010.
Neutral safety switch was not working when co-worker started super-4 with transmission in gear. Basket went into rib and he got hand caught between basket and fork mast smashing first 3 fingers on on left hand. He was taken to hospital and was not released back to work making this a lost time accident.

EE was taken to the ER and company doctor and was taken off work on 5/1/09 pending results of X-rays and follow-up visit.
Employee was riding on an open tractor through mine when debris went into his left eye. Employee was taken to company doctor and received anti-biotic drops. Employee was released to return to full duty.
Employee drove truck through chug holes and felt pain from neck between shoulder blades down back to belt line. Employee was taken to emergency room and was given an injection in neck.
EE was operating a forklift on the surface, EE went to adjust the left fork and it came completey off hitting top of EE's right foot. EE was seen by ER and released to CO. Dr. after x-rays it was revealed a possible fracture/contusion, referred to Ortho Dr. EE released to sedentary work only.
