As employee exited the stone pit in a loaded R35 Euclid rock truck via the haul road ramp the R35 rock truck stalled and coasted backwards down the ramp. The load shifted causing the R35 to roll onto the passenger side. Though the driver was kept in the seat by the wearing of his seatbelt his lower leg & foot were wedged between pedal & console causing injury.

THE WEIGHT OF ROLLER SHIFTED, CAUSING CHAIN TO SLIP. THIS CAUSED THE BOOM EXTENSION TO LURCH UPWARD, STRIKING EE UNDER THE BEEK OF HIS HARD HAT, CAUSING A LACERATION TO HIS FOREHEAD REQUIRING 16 STITCHES TO CLOSE.
