Employee was using a bobcat to lift an old pipe guard a foot off the ground and move it. Employee noticed that pipe guard started to slip so they went into reverse and the pipe fell out of the bucket and the bottom left leg of the pipe guard went into the cab of the bobcat and struck them in the left ring finger causing a laceration.

Showing all 19Employee was using a shuttle car to move two rail cars, due to wet slippery conditions shuttle car unable to stop. Shuttle car and two rail cars went to derail and ran off track. Employee worked for several days and did not request medical until 4/19/22 no restrictions given released full duty. Second Medical 4/22/2022 is when restrictions issued.
Employee was backing a WA600 Front-end loader when EE struck a pot hole causing a sudden jerk.
While driving a golf cart in the direction of the sun, he was blinded by the sun. He drove into a concrete column on the roadway.
Employee was walking out of silo #12 due south next to a parked truck getting loaded. The truck pulled out of the loading area, striking him on the left thigh.
Was using a Bobcat, cleaning the drainage area near the elbow dump area. When mud fall from his load hit the ground, some hot dust/steam came-up to his face and ears.
Dust blew into left eye, when he turned on the AC on the loader.
Injured was helping remove feeder wobble bars. He got his ring finger on his left hand caught in a pinch point. The pinch point crushed his distal phalange. It had to be amputated.
IP was attempting to reach behind a guard and hit a troughing idler with a hammer. IP does not remember exact details but sustained an injury when his LT hand passed between a troughing idler "can" and the running conveyor belt. IP's LT hand was bruised and he was placed on modified duty.
On 4/17 QC samples were being collected from a conveyor belt. Lockout requirements were not followed. The belt was accidentally activated by another employee for a few seconds. IP's knee contacted a conveyor x-member. No reportable injury was communicated. On 4/28, IP reported that while walking at a lake his knee gave out on 4/26. IP came to work on 4/28 on crutches & light duty.
Employee slammed shifter into reverse and jammed wrist.
Employee was operating a loader bucket. The loader bucket dropped to the gound by itself and it jerked the entire piece of equipment.
Employee did not know proper usage of air seat. Caused some soreness on tailbone,
Lost control of haul unit vehicle while driving to the pit.
EMPLOYEE WAS DRIVING HAUL TRUCK. WHEELS WENT ONTO BERM AND TRUCK TURNED OVER.
EE HIT A POTHOLE IN THE ROAD, THE EUCLID DOWNSHIFTED UNEXPECTEDLY & HE HIT HIS SHIN ON THE SHIFTER TOWER.
THE SHIFTER ON THE FRONT END LOADER BECAME STUCKEE BEGAN TO PUSH AND PULL THE SHIFTER PULLING A MUSCLE IN HIS LT. ARM.
PULLED NECK MUSCLE DRIVING ON ROUGHT PIT ROAD.
EE WAS ADJUSTING THE CLAY CHUTE ON PLANT WHEN HE SLIPPED AND TRYING TO CATCH HIMSELF PUT HIS LEFT HAND INTO HEAD PULLEY OF CONVEYOR.
