EMPLOYEE WAS DRIVING BEHIND TOWER ON HAUL ROAD WHEN HE HIT A DIP IN THE ROAD CAUSING HIS RT. KNEE TO HIT THE AIR CONDITIONER BOX & CAUSING A BONE CONTUSION. EE DID NOT WISH TO SEEK MEDICAL CARE UNTIL 10/25/05. HE WAS SENT TO THE DR. THE SAME DAY.
EMPLOYEE WAS OPERATING THE LOADER IN THE PIT. AS HE DUG INTO THE PILE, HE HIT A TOE IN ROCK CAUSING THE LOADER TO COME TO AN ABRUPT HALT. THIS CAUSED HIM TO WHIP FORWARD AND THEN BACKWARD IN THE SEAT.
Ee was hauling materials from south to north side of quarry in haul unit. As he was driving thru tunnel, bed was partially raised causing bed to hit roof of tunnel bringing him to an abrupt halt. As he was wearing his seatbelt this caused his face & shoulder area into the steering wheel causing lacerations to his tongue & lower lip, & cervical contusion.
EMPLOYEE WAS MOVING PLATE METAL (GROUNDMAN) WITH CHAIN ATTACHED TO LOADER BUCKET WHEN HOLE IN METAL BROKE, RELEASING CHAIN AND A PIECE OF METAL FELL ONTO HIM CAUSING A FRACTURED BONE IN LEFT S HOULDER BLADE/ FRACTURED RIGHT ANKLE AND BRUISEDLEFT LUNG.
