EE was operating a d400d haul truck hauling shot rock to the jaw. After he had dumped his load, he started to return to the pit location to be reloaded. While returning he struck a rough area on the haul road which caused him to jolted inside the cab causing a muscle strain back

Showing all 5The employee was moving a manlift into the pit to add a section of pipe to water pump. While moving the lift down the haul road on the east side of pit, the employee ran over pieces of shot rock that were approximately 8 to 10 inches in size, causing the manlift to rock back and forth resulting in the employee falling forward and striking his hand.
A FIELD CONVEYOR HAD STOPPED BECAUSE A ROCK GOT JAMMED IN THE TAIL PULLEY CAUSING THE CONVEYOR TO STOP. AFTER THE ROCK WAS CLEARED FROM THE TAIL PULLEY, EE NEEDED TO SHOVEL OFF CONVEYOR TO GET IT TO ROLL. WHILE SHOVELING ROCK OFF THE CONVEYOR, THE BELT STARTED TO ROLL BACK CAUSING HIM TO GET PUSHED TO ROCK BOX.
EMPLOYEE WAS CLEANING LOOSE MATERIAL AROUND QUARRY CRUSHER WITH BOBCAT WHEN A 6"ROCK FELL FROM THE CONVEYOR AND STRUCK EE ON THE LEFT KNEE CAUSING A CONTUSION.
EE DEVELOPED LUMBAR INFLAMMATION WHILE OPERATING HAUL TRUCK. EE IS ON MODIFIED DUTY THAT DOES NOT REQUIRE ANY SITTING FOR PERIODS OF TIME. WHEN BACK TO FULL DUTY WE WILL ROTATE EE INTO A PIECE OF EQUIPMENT THAT IS LESS STRENUOUS ON HIM.
