A ROLLER HAD BECOME STUCK DUE TO MATERIAL BUILDUP WHILE STRKING THE ROLLER WITH A HAMMER TO DISLODGE THE BUILT UP MATERIAL, THE EE LEFT HAND AND WRIST WAS PINCHED BETWEEN THE BELT & ROLLER. EE WAS CHECKED BY A DR AND RETD TOWORK ON MODIFIEDDUTY
EE WAS OPERATING LOADER ALL SHIFT AND WHEN HE WENT HOME HIS LOWER BACK TIGHTENED UP. THE AREAS HE WAS WORKING WAS HARD PACKED MATERIAL A FEW POT HOLES, WHICH MAY HAVE CAUSED HIM TO BE BOUNCED AROUND.
WHILE TRYING TO DUMP TRUCK THE PTO DID NOT ENGAGE ALL THE WAY. IT RAISED THE BED ABOUT 1 1/2". EE GOT OUT OF TRUCK AND KNELT BESIDE IT. HE PUT HIS LEFT HAND OUT TO SUPPROT HIMSELF WHILE TRYING TO KICK THE PTO IN. HIS HAND WAS BETWEEN THE BED AND THE FRAME, WHEN HE KICKED THE PTO IT DISENGAGED THE UNTI, LETTING THE BED COME DOWN ON HIS FINGERS.
FAILED TO FOLLOW DIRECTIONS ON MOVE SCRAPE MATERIAL WITH FORKLIFT. FORKLIFT UPSET AND WAS DAMAGED.
APPARENTLY EE REACHED IN FRONT OF THE FORKLIFT WHICH WAS GOING IN TO PICK UP A BARREL OF BALLS. EE WHO WAS INJURED WAS CAUGHT BETWEEN FORKLIFT AND BALL SORTER.
EMPLOYEE WAS LOADING TRUCK WITH PALLETS OF CEMENT. TRUCK DRIVER PULLED AWAY BEFORE EMPLOYEE WAS FINISHED LOADING WITH FORKLIFT. WHEN DRIVER PULLED AWAY, APPARENTLY UPSET FORKLIFT, EMPLOYEE THR OWN TO GROUND.
EMPLOYEE WAS TRAVELING TO WORK ON A MOTORCYCLE AND WAS APPARENTLY PROCEEDING AT AN EXCESSIVE SPEED. HE LOST CONTROL AND HIT BERM AND WOUND UP IN ASTRETCH OF DESERT. EXACT INJURIES HAVE NOT BEEN DETERMINED, EMLOYEE AIRLIFTED TO HOSPITAL.

UPON REALIZING THAT THE TRAIN DID NOT SEEM TO BE RESPONDING PROPERLY TO BRAKING AND REDUCING AIR PRESSURE, ENGINEMAN "DYNAMITED" THE TRAIN(SETTING ALL BRAKES). ENGINEMAN NOTIFIED INCOMING AREA PERSONNEL TO SWITCH TRACKS. BEFORE THIS COULD BEDONE, ENGINEMAN SAW THAT CRASH WAS EMINENT AND

EE WAS OPERATING 50 TONE TRUCK HAUL TRUCK INJURED ALLEGES HE LOST RETARDER AND BRAKES. HE PUT TRUCK INTO SIDE OF HILL TO STOP IT BUT THE TRUCK FLIPPED OVER. THE EE STATES HE IS HAVING A BACK P ROBLEM DUE TO THE ACCIDENT.

INJURED ATTEMPTED TO RIDE RAIL CAR OUT OF UNLOADING STATION IN VIOLATION OF SAFETY ORDER. WHEN HE REACHED EXIT, RUBBER CURTAIN KNOCKED HIM OFF THE CAR.
INJURED WAS USING A HAMMER IN AN EFFORT TO REPAIR A CONVEYOR. HAMMER COUGHT IN BETWEEN ROLLER AND BELT AND INJURED DID NOT LET GO.
ANOTHER EMPLOYEE JUMP STARTED A BOBCAT THAT WAS IN GEAR. THE BOBCAT TRAVELED APPROX. 20 FT AND HIT INJURED AND KNOCKED HIM DOWN.
INJURED WAS TO CLEAR A PLUGGED AIR SLIDE. SLIDE RELEASED SUDDENLY & LIMESTONE SPEWED OUT OF PORTS WHERE INSPECTION COVERS WERE REMOVED.
