The employee was checking the tightness of the belts when the machine activated, pinching employee's fingers between the belt and the pulley.

The employee was driving a haul truck, moving material around the plant,hit a rock in the roadway. The employee was not wearing a seat belt and as a result of hitting the rock, caused the truck to bounce employee up in the seat. When employee landed back in the seat, hurt back. This incident did not become a reportable injury until 5/9/2016 when lost time occurred.

The employee was operating a crane when it lost power, rolled backwards, hit a berm with the rear right tire and tilted on its side. The employee was injured by metal in the cab. The employee sustained a hand injury, sought medical attention and received stitches.

Employee undercut the highwall causing it to come down on the bucket jolting the loader. The employee was ok that night. Employee was sore the next day and asked for a few days off. He went to his doctor and could not identify any issues just sore and limitation of range of motion. The stiffness continued to get worse and we sent to workers compensation on 6/29/2012.

EE Directed the loader into position to load screen into bucket. signaled for loader to stop. walked in front of loader to drag screen into bucket when the operator lowered the bucket to the g round and EE's foot was under bucket tooth. crushing his left foot big and second toe. This incident caused both of the tips of his toes to be removed by the doctor.
