EE was pulling out from dumping a load of stone in a stock area. The Shift Supervisor pulled ahead of EE and in the exchange, EE forgot the bed was still up. EE pulled out and caught the raised bed in the top, throwing EE into the steering wheel. EE sustained a laceration to the forehead over EE's left eye.

Showing all 4Temporary employee was underground, operating the #16 haul truck. EE was backing under the loader when EE received first bucket load of 5th trip of the shift. The load shook the truck more than normal and EE felt neck tighten and pain shot through it.
Employee was cleaning with a bobcat loader and some heated dust blew out of the hopper and burnt his arm, face and neck.
WAS COMING OFF A COAL CAR. WHEN THE CAR STOPPED, IT JERKED THE CABLE HITTING HIS LEG AND CUTTING IT.
