A contractor employee was picking up a pallet of 50# bagged lime (2,500#s)w/a forklift. As he inserted fork lifts forks in pallet it fell forward on another contractor employee that was walking in the path of the falling pallet. Injured was air lifted to medical facility, released at 1:00 am after treatment and injury evaluation 8/7/07.

Showing all 3Employee was standing on a conveyor platform trying to free a loaded pallet of bagged product. His foot slipped and his left leg became trapped between a moving conveyor and a roller conveyor. He suffered abrasions and a crush injury to his lower left leg.
WHILE USING AN INDUSTRIAL VACUUM TRUCK, HIS LEFT ARM WAS PULLED INTO THE VACUUM HOSE CAUSING INJURY TO LEFT ARM. INJURED WAS ON REGULAR DUTY UP UNTIL 12/18/03 WHEN HE WAS PLACED ON MODIFIED DU TY DUE TO CONTINUING PAIN & DISCOMFORT RESULTING IN INJURY BEING MOVED TO REPORTABLE STATUS.
