Showing all 21Employee was placing a blasting cap in a drill hole with his left hand while holding on to the top rail of the loading basket with his right hand. The loading basket moved causing the basket to move against the mine face, mashing his finger between the mine face and the loading basket.
EE was cleaning tail pulley and had the #5 Kubota parked on the side of him. The Dubota started rolling, rearing him from the back and causing him to slip and fall, hurting his knees. EE was sent for med evaluation and released to regular duty. He continued to have pain and again sought med attention. ON 1/7/09 EE was diagnosed w/positive fracture to the left knee.
The employee was riding in a gator into the salt mine to fix a scaler. A piece of rock salt fell from above and broke his right femar and fractured bones below the knee cap.
Employee was traveling along the conveyor belt in a Kubota ATV, when one of the front wheels struck a chunk of material in the roadway. This caused the steering wheel to turn abruptly and his elbow was struck by one of the steering wheel spokes. He continued working his normal shift/duties, but as of 4/25/08 he has been placed on restricted duty by his physician.
Employee was injured while driving a golf cart along the conveyor belt and fell asleep, steering into the conveyor belt structure. He awoke just as he struck the conveyor structure. The employee was working "over" 4 hours in addition to his regular shift.
EE states he was preparing to move a load of round cans using the Nissan electric fork truck. When he applied pressure on the operating handle without activating the speed control, the machine abruptly moved forward pulling the employee with his left arm resulting in a shoulder strain.
Employee was elevating a high-lift platform to weld beam being held by C-clamps. As he raised basket, not observing hand position, he caught hand/thumb between basket railing and I-beam he was to weld. Employee suffered minor fracture to distal end of left thumb. Reported as Medical Treatment/Restricted Duty.
Employee states he was operating a fork truck unloading a pallet in the warehouse. While backing out of the pallet rows, he was struck by another fork truck that was traveling from the dock area. He was seen by a physician and released for full duty. On April 30, 2007 he went back to the doctor and was not released to return to work.
Employee started to tram vehicle in reverse for another to pass; was closer to rib than realized; vehicle struck rib; employee jarred and struck metal structure near operator compartment. Incident reported as medical treatment - sutures.
EE states he injured his back while operating a fork truck or picking up a 50 lb bag of salt. EE was seen by a physician on 3/27/06 & had X-rays. EE was instructed to remain off work until X-rays read. Physician was away from the office through 3/30/06. Diagnosed with siatica & EE instructed to remain off work and follow up visit on 4/10/06.
EMPLOYEE STATES HE WAS TRANSPORTING PELLET PALLETS TO SOUTH DOCK STORAGE AREA WHEN THE FORK TRUCK HE WAS OPERATING CONTACTED A DEPRESSION IN THE ASPHALT PAVEMENT. THE CONTACT WITH THE DEPRESSI ON RESULTED IN A SUDDEN TWISTING MOVEMENT OF HIS UPPER BODY THAT RESULTED IN LOWER BACK PAIN AND DISCOMFORT.
WHILE LOADING A PACKAGE VAN TRAILER WITH NISSAN FORK TRUCK, EE CONTACTED A DEPRESSION IN THE TRAILER FLOOR WITH THE RIGHT FRONT WHEEL OF THE FORK TRUCK AND CAUSED A JOLTING MOVEMENT TO THE FOR K TRUCK. EE REPORTS DISCOMFORT IN HIS UPPER BODY AND LOW BACK.
EE WAS RETURNING FROM BREAK. EE STEPPED INTO PATH OF FORK TRUCK. FORK TRUCK OPERATOR WAS UNABLE TO AVOID EE. THE FORK TRUCK AND EE COLLIDED. EE SUFFERED TORN LIGAMENT IN LEFT KNEE. INSPECTION OF FORK TRUCK REVEALED NO DEFECTS.
EE STEERED ATV FULL RIGHT & STEERING LOCKED. TO AVOID MACHINE TIPPING OVER, HE USED LEFT FOOT ON GROUND TO STABILIZE THE UNIT WHILE STOPPING. THE MAIN FRAME BROKE CAUSING STEERING TO LOCK. INI TIAL MEDICAL FINDINGS WERE NEGATIVE BUT LATER MRI REVEALED NECESSITY FOR SURGERY. EE PERFORMED FULL JOB DUTIES PRIOR TO SURGERY, WHICH OCCURRED ON 5/9/03, THEN HE WAS PLACED ON RESTRICTIONS.
EE WAS CLEANING TAILPIECE OF BELT CONVEYOR WHEN SHE NOTICED ONE OF THE EXPANDED METAL GUARDS WASOUT OF POSITION. THE EE GRABBED THE EDGE OF THE GUARD AND ATTEMPTED TO MOVE IT. 2 OF THE EE'S FINGERS WERE CUT AND ONE WAS BROKEN.
WHILE WALKING BACK TO THE SUPER SACK MACHINE, EEWAS STRUCK ON THE REAR OF HIS RIGHT FOOT BY THE REAR WHEEL OF A NISSAN FORK TRUCK.
