OE was attempting to move a roll of pond liner with a fork lift near railroad tracks. The forks became stuck on the tracks. When backing up, the forks popped off the ground and the liner rolled off of the forks down a slight slope about 3 feet. EE was standing in the path of the rolling liner and it rolled over ankle. EE suffered a small fracture on foot and ankle.

The ee in question was assisting a coworker in releasing a tire trailer from the fifth wheel coupling on a tractor. The employee managed to get the pin to release and instructed his coworker to move the tractor forward. The EE in question was not aware of his body positioning, had his foot in the front of the tractor's rear tires, resulting in his foot getting run over.

An employee with 12 years experience was reclaiming clinker, when he came in contact with hot material, causing 2nd and 2nd and a half degree burns to 15% of his upper extremities.

Left hand pinkie finger was caught between the safety hand rail of a man lift and the equipment cross beam while cleaning up fugitive dust and buildup. Finger was lacerated, 19 stitches required. Possible partially torn tendon in finger. Employee referred to specialist for further treatment.

Driver injured when seat of loader broke, pushing her forward in the cab and held her in an awkward position. The welds holding the seat had broken in two spots, allowing the scissor mechanism to force it forward. The driver requested no medical help or treatment at the time of the injury.

Employee approached a haul truck parked at the ready line. He lifted his left foot to step onto the lower step of the access ladder, and felt a `pop' in his right knee. He immediately stopped on the ladder, and waited for the situation to feel better. After a few moments, he continued on up the ladder, and completed the work on the door, and then exited the truck.

While loading clinker dust with a skidsteer, dust blew into EE's eyes, causing some light irritation and redness. By end of shift, EE reported his eyes were itchy/watery/sore/red, but felt okay to drive home. Next morning, EE reported eyes were still sore, inflammed/red, and was sent to doctor for check.

WHILE CLEANING UP A COAL SPILL, THE EMPLOYEE WAS CLEANING THE RAILS & WHEN APPROACHING THE UNLOADER THE BUCKET HIT A RAISED PART OF THE GRIZZLY. THIS THREW THE EE INTO THE SAFETY BAR OF THE BOBCAT HITTING HIS CHEST, WHERE HIS RADIO MIKE WAS.

EE WAS OPERATING A 992G LOADER. WHILE DIGGING, BUCKET CAME IN CONTACT WITH A LARGE ROCK CAUSING REAR OF LOADER TO COME OFF THE GROUND. WHEN REAR OF LOADER RE-CONTACTED THE GROUND, EE WAS BOUNC ED IN THE SEAT CAUSING LOW BACK PAIN. EE IS SHORT & LIGHTWEIGHT & MUST ADJUST SEAT TO LOWEST POSITION TO REACH PEDALS. BY DOING THIS CAUSES THE SEAT TO LOSE ITS SHOCK ABSORBING CAPABILITIES.

EMPLOYEE WAS DUMPING MATERIAL INTO A SMALL DUMP TRUCK WITH A BOBCAT. WHILE RAISING THE BUCKET AND MOVING FORWARD TO DUMP, THE BOBCAT STARTED TO BUCK (ROCK BACK & FORTH), CAUSING THE BOBCAT TO TIP FORWARD. IT APPEARS THAT THE EMPLOYEE LET GO OF THE CONTROLS AND REACHED OUT, PLACING HIS HANDS ON THE SIDE OF THE DUMP BED. THE CAB OF THE BOBCAT THEN CAME DOWN ON THE EMPLOYEE'S HANDS.
