Employee was helping a contractor load and move out their welding machine on the contractor's forklift.The welder was not centered very well on the forks, and when it was picked up approx. 15" it started to fall. He stepped forward to help steady the welder and at the same time the forklift operator lowered the forks, setting the fork on his foot.

PROBLEM WITH FORK LIFT OVER HEATING. EE WAS CHECKING RADIATOR HOSE WITH FORKLIFT RUNNING. APPARENTLY FINGER WAS CAUGHT IN BELT PULLEY.
