Showing all 34On 4/16, a loader operator was working all day on the loader. EE indicated that EE did not want to be on the loader the next day due to the seat hurting EE's back. On 5/3 EE returned to the clinic and was issued restrictions.
Operator stated that they had hurt their neck when they looked up and down while traveling up Pushback 82 ramp on the previous shift. EE was issued restrictions.
Haul truck operator was queued up at the crusher. A loader operator had begun clean up on the floor in back of the HT. The loader had backed into the parked HT bed. The HT operator felt some discomfort in lower back as the night went on. The haul truck operator was taken to seek medical attention and was issued restrictions.
EE was working in removing petron barrels. EE was helping a skidsteer operator by holding & guiding the barrel into the bucket of the skidsteer. The barrel started to tilt over & to prevent it from falling the skidsteer operator put the bucket down, pinching the EE's foot. EE was take to clinic & issued restrictions.
An operator of the manvan was going too fast & hit a washout behind the Crusher tiedown. This caused the EE in the back seat to be bounced up & down. EE's back & neck became jarred. As they were headed back to Room 100 once again the EE became jarred by the speed bump. EE sustained neck & back strain. Restrictions issued.
An operator was backing up to a shovel to get loaded. Operator pulled forward to realign with dipper. As they started to back up again the operator felt contact with shovel that was on the left side of the shovel. EE in shovel felt some pain in neck like whiplash. EE was issued restrictions.
EE was operating Haul Truck 65 & pre-spotted on the right side of 43 shovel. Shovel operator stated that HT 65 was too far out & at an angle. Shovel operator released the trip & the corner of the dipper door hit the side of the bed. HT operator began to feel sharp pain in back area when this happened. EE was taken off work due to muscle strain.
EE was being loaded at the 43 shovel when a large boulder was dropped in EE's haul truck. The truck shook and the EE felt a pinched nerve/muscle in EE's neck along with EE's thumb going numb. EE was taken to the clinic the next morning and issue restrictions.
On 09/24/2020 an operator was loading with a loader and jarred self on some heavy rock causing lower back pain. EE did not inform site. On 10/06/2020 EE notified supervision of lower back pain. Was issued restrictions on 10/06/2020.
A haul truck was traveling down from Pushback 61 ramp & lost traction. The haul truck began to fishtail & it clipped another haul truck traveling up the ramp loaded. The EE felt soreness and tightness in back the next day when they woke up. EE was taken to the clinic & issued restrictions.
On 12/04/2019 EE of pickup truck was traveling up a haul ramp & for unknown reasons ran pickup truck into toe of highwall. Due to large material at toe it caused extensive damage to pickup truck. On 12/19/2019 EE was prescribed Physical Therapy for stiffness & sore neck and back.
EE was shoving a pile from the middle of the 7B & 8B belts. EE had changed positions of shoveling & got on knees. EE picked up a shovel load & pulled it back to dump when the shovel handle got caught on the 7B return idler & pulled the shovel out of the EE's hand, striking EE on the forearm. The EE had a right arm contusion.
EE was being loaded at 48 shovel. While driving to V dump to dump load EE felt something go into EE's eye. EE was taken to clinic to be medically treated. Multiple treatments for scratched cornea with antibiotics.
EE was in EE's haul truck getting loaded. A boulder was in the dipper. As the shovel operator was loading the truck a boulder came into contact with the bed of the haul truck causing the ee to get jarred. Stated that EE's back hurt.
EE was removing shovel cable from front of light vehicle. Operator of vehicle moved forward & made contact with the EE who was removing the cable. EE didn't fall as a result of the vehicle but as the vehicle proceeded forward & was still pulling the cable, the cable made contact with the EE which caused EE to fall. EE sustained multiple contusions.
Truck driver was injured when hydraulic cylinder failed and bed dropped on truck frame while unloading. Driver was taken to clinic and released with no restrictions. 8 days later driver reported headache and neck stiffness. 2nd visit to clinic resulted in duty restrictions.
EE was working on Haul Truck 39 repairing a steering/hydraulic leak. EE was reaching to get tools from underneath the truck; he struck the right side of his head with the steering tie rod, creating a strain in his neck.
EE was sitting in CAT 30, and was being loaded by loader operator. Loader operator was easing a large rock into the back of CAT 30. The rock dislodged from the bucket and created a concussio n in back of truck, jarring EE.
Employee was driving out from dumping load on West side of #3 drop box onto Duval Mine Road when bed of truck hit the trestle overhead causing dump truck to tip over.
Employee alleges that he injured his tailbone and lower back as a result of an air suspended haul truck seat bottoming out. Employee was taken to the clinic and prescribed muscle relaxants and put on restricted work duty. .........Pending further investigation...........
Employee was workiing on clean up under 7-B conveyor by the tail end when he lost sight of the end of the hoe. The hoe climbed up the pile of rocks he was working on causing it to come in contact with the return idler pulling the tool from his hands. As he released the tool, his hand struck the support beam that was in close proximity.
Employee was holding a stand serving as a base for a lightning rod. The base of the stand was being covered with dirt by a loader when a rock rolled out of the loader bucket and smashed his index finger against the stand.
As employee was leaving the parking lot at the end of his shift, the motorcycle he was operating was struck by a contractor's employee backing out of his parking spot without clear visability.
Employee inhaled coolant fumes when the line broke in the haul truck.
On April 29, 2008, employee inhaled haul truck coolant fumes. He felt no immediate ill effects and remained working normal shifts. On 5/16/08, employee requested to be evaluated by medical personnel since he feels he is now feeling ill fom the exposure.
Pickup contacted berm and rolled over.
Employee was compacting earth with a compactor, stepped backward and stepped on an exposed section of pipe. The employee slipped and fell backward, the compactor fell one way and the employee fell the other way. He reported the injury the next day, compalining of knee pain.
Employee alleges the air seat in the haul truck bottomed out while driving over rough road in the mine resulting in a strain to the lower back.
Employee was attempting to adjust the forks on the forklift when his hand became caught between the mast and the fork causing a contusion to his right hand.
A truck was returning to maint. shop from the mill heaping east on south side of maint. shop. EE was operating a forklift hauling west. The truck was on the S. side of the hydrant and did not see the forklift. Both were still moving when they hit. Trying to reach Dr. for claificatio of discrepancy between written report and release he gave EE. D&A performed on 4/6/06 were neg.
Hoist brakes on shovel did not hold dipper as it was loading the truck. The dipper fell and struck the bed of the truck, raising the bed into the air and jarring the truck driver. NOTE: Employee continued to feel pain, therefore he was sent for re-exam. This exam revealed a need for physical therapy elevating this incident from first aid to medical treatment.
EE WAS COMING OUT OF #5 MILL ON A LINER BELT. HIS LEG WAS PINCHED BETWEEN THE BELT AND STEEL FRAME RESULTING IN A CRUSH CONTUSION TOHIS RIGHT UPPE LEG.
EMPLOYEES WERE REPLACING LINERS FOR THE FEEDER. EE WAS EXITING THE SURGE POCKET OF THE FEEDER BELT CRAWLING ON HIS HANDS AND KNEES WHEN SOME MATERIAL DISLODGED COMING DOWN AND COVERING BOTH HI S LEGS APPROXIMATELY TO MID-THIGH. THE FORCE OF THE MATERIAL CAUSED EMPLOYEE TO FALL FORWARD. HIS LEFT HAND SLID, CAUGHT ON THE BELT RUBBER SURFACE & THEN ROLLED UNDER HIS WRIST CAUSING INJUR
