Employee applied light pressure to the aerial lift joystick expecting to raise the basket just a few inches. The lift responded to the controls by abruptly raising at least a foot crushing the employee's hand between the lift basket and an I beam. Employee received severe lacerations and a joint fracture to the little finger on EE's right hand.
The driver was driving too fast for conditions which was the root cause for this accident.
The employee reached into a moving conveyor to attempt to remove an empty bag that was caught in the roller. While attempting to remove the bag, his arm was pulled into the conveyor.
The employee was hauling clay in a dump truck and while negotiating a 90 degree right turn the truck turned over on its side. Speed was a contributing factor. The employee sustained a laceration near her eye and on her arm.
Employee was adjusting forks on forklift when his right ring finger got caught between the fork and mast. The end of the finger was severed off.

TWO TRACTOR TRAILERS COLLIDED ON TUCKER ROAD HEAD ON
DRIVER WAS DUMPING LOAD OF CLAY AND TRAILER & TRUCK ROLLED OVER ON LEFT SIDE INJURING DRIVER WHO HAD TO BE TRANSPORTED BY AMBULANCE TO MEDICAL CENTER. DIAGNOSIS WAS A CERVICAL STRAIN. PRESC RIBED ICE TO SHOULDER/NECK/PELVIS FOR 1ST 24 HRS, THEN HEAT; WEAR SLING FOR 3-4 DAYS/MEDICATIONS AS PRESCRIBED & FOLLOW UP WITH MD IN A FEW DAYS.
WHILE OPERATING 50 TON OFF ROAD TRUCK HE STARTEDEXPERIENCING PAIN IN RIGHT SIDE AND FELT NUMB INTHE SAME AREA. HE REPORTED ON 9/26 AFTER SEEING DOCTOR ON 9/19 AND OUT OF WORK SINCE. HE INDICAT ED CURRENT WORK CONDITIONS CONTRIBUTED TO PAIN. IN DOCTOR'S REPORT HE DID NOT INDICATE THIS AS AWORK-RELATED INJURY. DELAY IN REPORTING WAS DUE TO THE DOCTOR'S FINDINGS.
BACKING UP TO LOADER FOR LOADING, LOADER OPERATOR STRUCK TRUCK BODY WITH BUCKET, JARING OPERATOR IN CAB OF TRUCK CAUSING NECK INJURY-STRAINED MUSCLE.
THE RIGHT REAR TIRE OF THE HAUL TRUCK DROPPED INTO A 3.5 FOOT DROP OFF CAUSING THE FRONT END OF THE TRUCK TO RISE UP INTO THE AIR. THE TRUCK THEN ROLLED ONTO ITS RIGHT SIDE. THE VICTIM'S HEAD STRUCK THE DOOR FRAME CAUSING INJURY.
TRAVELING ON MINE DIRT ROAD, HIT A BUMP CAUSING LEFT KNEE TO HIT STEERING COLUMN; KNOT ON KNEE, SOUGHT MEDICAL ATTENTION. LOSS TIME.
