Forklift was parked, the operator climbed in to begin another task. Upon turning on the forklift the forklift rolled and rolled over injured employee's right foot. This forklift is a hydrostatic brake system, when you let off the gas the forklift is to stop.

Employee stated he has been running a loader for 35 years and am 59 yrs old and the right side of my neck started to hurt and become stiff. Tense muscles in neck. NO restrictions but started physical therapy on July 15th to ease muscles. I received the list of therapy sessions on July 29th.

Driving a loader down haul road and saw a rock. He lowered the bucket to scoop up the rock and forgot to check if the bucket was level. The bucket dug into the road and abruptly stopped the truck. EE hit the windshield of the loader.

Employees were installing a new conveyor belt. The belt was being pulled with a loader and cable. Traffic was being stopped as the loader approached a road. The cable started to rub on a road sign. Employee attempted to get the cable clamps past the sign when the tension on the cable knocked the sign over striking the employee on the left leg.

Injured employee was stacking brick onto a pallet one at a time by hand inside kiln #6. The forklift operator moved to pick up the pallet, not seeing the employee, and the fork pushed the injured employee's left foot into the pallet. This caused an approx. 1 inch laceration to the top of the injured employee's left foot.

EMPLOYEE SAYS HIS LEFT WRIST HURTS FROM RUNNING THE CONTROLS ON THE LOADER. HE REPORTED THIS TO ME ON 2/3/04. HE WENT TO THE RAPID CARE AND SAW A DOCTOR. HE SAID TO TRY IBUPROFEN & HEAT & COLD .HE RETURNED TO RAPID CARE ON 2/20/04 & WAS REFERRED TO THE PHYSICAL THERAPIST AT PT-OT PROFESSIONALS FOR THERAPY 3 TIMES A WEEK FRO 4 WEEKS. NO WORK RESTRICTIONS.
