Miner went into c-9 tunnel conveyor to unjam feeder. At this time the cause of the incident can't be determined. Management and the MSHA inspector could not interview the miner due to injury. Miner lost left arm in the tail pulley. At this time why miner was at the tail pulley is unknown.

EE WAS SAID TO HAVE HAD A MILD HEART ATTACK CAUSING HIM TO BLACK OUT. DURING THE BLACK OUT PERIOD, THE EE DROVE THE HAUL TRUCK THROUGH A ROCK BERM AND FLIPPING OVER 20 FEET BELOW THE HAUL ROAD . THE EE SUFFERED 2 FRACTURED NECK VERTABRA AND THREE BROKEN RIBS.
EE WAS OPERATING BACKHOE (SCOOP IN FRT) WHEN HE FELT SOMETHING IN HIS EYE. HIS FOOT WAS ON BRAKEBUT SLIPPED WHEN HE FELT SOMETING IN EYE MACHINEMOVED FORWARD STRIKING A PIECE OF PROTRUDING MET AL THAT SHATTERED THE GLASS EE HAD SOMETHING IN LEFT EYE & WAS WEARING SAFETY GLASSES AT THAT TIME
EMPLOYEE CLIMBED DOWN THE LADDER FROM THE 36" CONVEYOR AND TURNED TO WALK AWAY WHEN HE WAS STRUCK ON THE RIGHT SHOULDER FROM A ROCK THAT CAROONED OFF OF THE CONVEYOR.
