Employee Claims: While operating a front-end loader and digging from a muck pile, a rock was thrown from the "Surestrike" rock breaker through the right side window of the loader. The rock struck the employee on the right side of the face.

Employee Claims: "From taking bumps in the loader. Sometimes the bucket of the loader hits the wall and jerks you around pretty hard. I started to feel pain in my neck/back." We were notified by the employee that he was having health problems on 09/03/04. The employee then notified us on 10/07/04 that the health problems were work related.
