Employee was driving a maintenance truck down the quarry road when he lost control of the truck & it flipped. He was trapped in the vehicle until emergency response personnel retrieved him. He received stitches in his ear & was bruised. He was seen at the ER & discharged on same day. He was instructed to not return to work until results of next checkup on 5-13-09.

AFTER DUMPING A LOAD OF ROCK, EMPLOYEE FAILED TO LOWER BED ON ROCK TRUCK. THE RAISED BED STRUCK A CROSS BEAM OF A STEEL STRUCTURE AS EMPLOYEE DROVE THE TRUCK, ABRUPTLY STOPPING THE TRUCK. DESP ITE WEARING SEAT BELT, EMPLOYEE WAS INJURED WHEN HE HIT THE STEERING WHEEL.
EE WAS SITTING IN HAUL TRUCK. A LOADER OPERATOR WAS LOADING THE ROCK TRUCK WITH ROCK WHEN THE LOADER ACCIDENTALLY BUMPER THE ROCK TRUCK BED. AS A RESULT OF THIS CONTACT, THE EE REPORTED A STRA INED RIGHT SHOULDER.

EE WAS RAISING THE WINDOW IN THE TRUCK WHEN THE LEVER TO CONTROL THE RAISING LOWERING OF THE WINDOW WAS RELEASED CAUSING THE WINDOW TO FALL BACK ON HIS RIGHT INDEX FINGER THE RIGHT INDEX FINGE R WAS CUT AND REQUIRED EIGHT STITCHES.
