INJURED WS TRAVELING FROM TEH 1650 SHOVEL COAL PIT IN AMACH COAL TRUCK (LOADED). HE TRIED TO NEGOTIATE A SHARP RIGHT HAND TURN. WHILE MAKING THE TURN, HIS TRAILER JACK KNIFED AND TURNED THE ENTIRE TRUCK OVER (DRIVER'S SIDE). INJURED CLIMBED OUT OF THE FRONT WINDWSHIELD AND WALKED AWAY FROM.

EMPL WAS DUMPING LOAD OF REFUSE ROCK ON HAUL ROAD. TRUCK OBVIOUSLY BECAME UNBALANCED & FLIPPED OVER ON PASSENGER SIDE OF TRUCK. EMPL FELL FROM DRIVER SEAT TO PASSENGER SIDE OF CAB. HEAD CRACKE D PASSENGER SIDE WINDOW.
WAS DRIVING OUR FUEL TRUCK ON MINE PROPERTY WHEN HE HIT A BUMP. THIS BUMP CAUSED THE STEERING WHEEL TOS JERK ABRUPTLY. THE JERK OF THE STEERING WHEEL CAUGHT HIS LEFT THUMB AND DISLOCATED IT. ON 2-16-91, HE VISITIED DR IN O'BORO. HE WILL BE OFF FOR 6 DAYS.
EE WAS HAULING GOB FROM OUR E 60 PREP PLANT AND DUMPING IT IN A GOB PILE LOCATED ON THE COAL YARD. HE WAS OPERATING AN UNFAMILAIR TRUCK. WHEN HE RELEASED THE CHAIN THAT HOLDS THE TAILGATE LEVE R THE LEVER SPRING.
EE WAS HELPING MOVE TRACTOR THAT WAS HUNG UP AND SPINNING, HE WAS KICKING ROCKS UNDER THE SPINNING TIRE TO GET BETTER TRACTION WHEN HIS FOOT CAUGHT UNDER TIRE AND INJURIED RIGHT FOOT JUST BEYO ND STEEL CAP.
INJURED WAS IN THE PROCESS OF DUMPING A LOAD OF FINES BACK ON THE YARD. HE RAISED HIS TRAILER AND DUMPED A SMALL PORTION. AFTER THIS SMALL AMOUNT WAS RELEASED, HE DECIDED TO MOVE HIS TRUCK I NTO ANOTHER POSITION. FORGETTING TO LOWER HIS TRAILER. HE PULLED FORWARD WITH HIS RIGHT FRONT TIRE ROLLING INTO A HOLE. WITH HIS TRAILER PARTIALLY RAISED AND ALMOST FULLY LOADED THIS UNBALA
EMPLOYEE WAS BACKING 2 TON TRUCK IN PIT AND HIT A LARGE ROCK THAT WAS PARTLY HIDDEN IN MUD AND CAME TO SUDDEN STOP AND HURT NECK. HE WENT TO DOCTOR AFTER WORK AND RETURNED TO WORK THE FOLLOWIG N DAY AND WORKED EVERY DAY 7 DAYS A WEEK UNTIL 8-5-89 AND HAS BEEN OFF FROM WORK SINCE. HE IS CLAIMING WHIPLASH TYPE INJURY TO NECK.
WHILE OPERATING A FRONTEND LOADER HE HIT A HOLE IN THE YARD THAT BOUNCED HIM IN THE SEAT HE STARTED HAVING BACK PAIN AND HE WENT TO A CHIROPROCTOR HIS VERTEBRATE WAS OUT OF ALIGN IN SIX PLACES AND THE CHIROPRACTOR REALIZED IT.
WHILE OPERATING AN ENDLOADER DEBRIS BLEW IN OPERATORS EYE.
WHILE OPERATING A SCRAPER THE OPERATOR HIT A ROCK AND HIS SEAT BOTTOMED OUT WHEN HE BOUNCED. HIS BACK STARTED TO HURT AND HE WENT TO A CHIROPRACTOR. HIS LOWER BACK WAS OUT OF PLACE AND THE CHIROPRACTOR RELOCATED IT.
WORKING WITH A GALLION 125 TO HOIST A SKIPBOX ATTACHED TO THE HOISTING HOOK BLOCK THE EMPLOYEE WAS WORKING 30 FT IN THE AIR PREPARING THE POSTS FOR A PREFABRICATED CONVEYOR SYSTEM WHEN THE HO ISTING HOOK BLOCK CAME IN CONTACT WITH THE BOOM CAUSING THE HOOK TO TILT TO AN ANGLE SO THAT TWO OF THE CABLES CAME UNHOOKED DROPPING ONE END OF THE BOX AND THE EMPLOYEE FELL TO THE GROUND

HIS RIGHT ARM IN THE STEERING WHEEL OUT OF HIS HAND CATCHING HIS RIGHT ARM IN THE STEERING WHEEL FOREMAN WAS NOT NOTIFIED UNTIL 6N THE STEERING 14-88 THIS IS WHY IT TOOK LONGER THAN 10 DAYS TO REPORT WENT TO THE DR ON 6-14-88 AND WAS GIVEN MEDICAINE FOR PAIN.
HE WAS DRIVING 5 FRONT END LOADER ACROSS THE YARD AND HIT A HOLE THAT JARRED HIM AND HIS BACK STARTED HURTING HIM.
