HE WAS STANDING IN A CROSSCUT ADJACENT TO A HAULAGE ROADWAY. A SCOOP TRAVELING THE ROADWAY RUN OVER A PIECE OF COAL WHICH BURSTED AND A PIECE OF THE LUMP OF COAL STRUCK HIM ON THE FOREHEAD.
