THE SHIRT CAUGHT ON SOMETHING HANGING FROM THE MINE ROOF AND STARTED PULLING HIM BACKWARDS AT WHICH TIME HIS FOOT GOT CAUGHT UNDER THE FRONT OF THE CAR THAT HE WAS UNCOUPLING CAUSING HIM TO TWIST HIS LEFT KNEE. (I.NAME WAS SITTING ON SUPPLY CAR WHILE HE WAS UNCOUPLING IT.)
MECHANIC WAS WORKING ON #25 SHUTTLE CAR RUNNING PUMP MOTOR IN ORDER TO TURN WHEELS TO GET TO STEERING ARM, WHEN HE ACCIDENTLY STRUCK TRAM PEDAL CAUSING SHUTTLE CAR TO TRAM, PINNING WORKER BETW EEN FRAME OF SHUTTLE CAR AND RIB.
