OPERATOR WAS HOOKING MINE TRACTOR TO MINE CAR. THE OPERATING CONTROLS WERE FROZEN WITH MUD AND GRIME. OPERATOR WAS IN THE PROCESS OF CLEANING THE CONTROLS WITH POWER ON. TRACTOR MOVED IN REVER , CRUSHING HIS FOOT BETWEEN MINE CAR AND TRACTOR ASSUMPTION.
