EMPLOYEE WAS RIDING IN A PICK UP TRUCK WHEN HE FELT LIKE THERE WAS SOMETHING IN EYE. LATER, HE LOOKED IN A MIRROR AND IT APPEARED HE HAD A BLISTER IN HIS EYE. HE WAS CHECKED BY A MEDICAL DOCTO R AND AN OPTOMETRIST, GIVEN MEDICATION FOR THE CONDITION AND RETURNED MTO WORK WITHOUT RESTRICTION. THE CAUSE OF THE ONSET REMAINS UNKNOWN.

EE IS A UTILITY OPERATOR AND AT THE TIME OF THE REPORTED INJURY WAS OPERATING A CH 120 EUCLID COAL TRUCK. HE HAS STATED THAT DURING THE TIME OF 3/8,9,10/2000 THE SEAT IN THE TRUCK WOULD NOT AD JUST CORRECTLY AND CAUSED INJURY TO HIS LOW BACK. NO REPORT WAS MADE TO THE COMPANY UNTIL HE MISSED WORK ON 3/20/00. AT THAT TIME HE FILED AN INJURY REPORT.
