THE EE & 2 COWORKERS WERE ROCKDUSTING ALONG THE #2 BELT UTILIZING A 12 TON LOCOMOTIVE AND A TRACK DUSTER. THE EE PARKED THE LOCOMOTIVE ON A GRADE SET THE PARK BRAKE AND EXITED THE OPERATORS COMPARTMENT TO ASSIST IN PUTTING ROCK DUST IN THE MACHINE. HE WAS STANDING WITH BOTH FEET ON THERAIL (1 ON EITHER SIDE OF THE RIGHT SIDE WHEEL NEAR THE LOCOMOTIVE) THE ROCKDUSTER AND/OR LOCOMO
EE WAS DUMPING A LOAD OF COAL, COAL STRUCK IN THE TRAILER AS IT WAS BEING RAISED, CAUSING TRACTOR AND TRAILER TO TURN OVER.
THE EE WAS OPERATING A MANBUS ON THE MIAN LINE TRACK. HE WAS COMING OFF AN INCLINE AREA INTO A DIP WHEN THE MANBUS DERAILED THROWING HIM INTO THE CANOPY SUPPRT. THIS RESULTED IN A HAIRLINE FRA CTURE TO ONE OF HIS RIBS ON HIS RIGHT SIDE
EE WAS HAULING COAL FROM THE FACE TO THE FEEDER USING A JOY 10SC END DRIVE SHUTTLE CAR. WHILE TRAMMING TOWARD THE FEEDER, THE EE REPORTS THAT THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY CAUSING HIM TOBE THROWN INTO THE UNDERSIDE OF THE OPERATOR'S CANOPY. THE OPERATOR SUFFERED A SPRAINED NECK AS A RESULT.
THE EE WAS HOLDING UP THE MINER CABLE AND WATER LINE FOR A SHUTTLE CAR TO PASS THRU A CROSSCUT WHEN THE ACCIDENT OCCURRED. EE RIGHT HAND WAS CAUGHT BETWEEN THE SHUTTLE CAR CANOPY AND THE MINER CABLE WHEN THE SHUTTLE CAR PASSED UNDERNEATH. THIS RESULTED IN A LACERATION TO HIS RIGHT INDEX FINGER. SUTURES WERE USED TO CLOSE THE WOUND. THE EE WAS WEARING METACARPAL GLOVES. NO LOST TIME
EE WAS OPERATING A JOY 10SC SHUTTLE CAR HAULING COAL FROM THE FACE TO THE FEEDER & RETURN. WHILE TRAMMING THE SHUTTLE CAR IN THE R CROSSCUT OFF #5 ENTRY [STATION SPAD #14884], THE EE REPORTED THAT THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY & CAUSED HIM TO BE JOLTED IN HIS SEAT. HE REPORTED EXPERIENCING PAIN IN HIS NECK & LOWER BACK. AREA HEIGHT 68", DRY BOTTOM. EE OFF 8-8-9

AS HE WAS TRAMMING SHUTTLE LOADED FROM FACE TO FEEDER, SHUTTLE STRUCK A DEPRESSION IN ROADWAY(#7 ENTRY) AND CAUS ED HIM TO BE THROWN AGAINST UNDERSIDE OF CANOPY OF OPERATORS COMPARTMENT. EE REPORTED THAT HE EXPERIENCED PAIN IN HIS NECK AS A RESULT.
EE REPORTED THAT AS HE WAS TRAMMING AN EMPTY JOY 10 SC SHUTTLE CAR FROM THE BELT FEEDER TO THE FACE, THE SHUTTLE CAR STRUCK A DEPRESSION IN THEROADWAY AND JOLTED HIM IN THE OPERATOR SEAT. HE R EPORTED HAVING PAIN IN HIS NECK AT THE END OF HIS SHIFT. THE ACCIDENT OCCURRED AT SPAD 14008/1 BREAK INBY.'

EEREPORTED HE WAS JOLTED AND THAT HIS HARD HAT STRUCK THE UNDERSIDE OF THE OPERATOR'S CANOPY RESULTING IN A SUDDEN PAIN IN HIS NECK.

THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY (#7 ENTRY) AND THE EE WAS JOLTED IN THE OPERATOR'S COMPARTMENT. HE REPORTED EXPERIENCING PAIN IN HIS LOWER BACK.
EMPLOYEE WAS ON FOOT WALKING THROUGH THE FIRST CROSSCUT INBY THE BELT FEEDER A JOY 10SC SHUTTLE CAR TRAVELING DOWN THE BELT ENTRY TOWARD THE FEEDER PASSED THE INTERSECTION ADJACENT TO THE CROS SCUT AS THE EMPLOYEE ATTEMPTED TO STEP ACROSS THE TRAZAILING CABLE IT CAUGHT HIS LEGS & DRAGGED HIM ACROSS THE CROSSCUT & AGAINST THE COAL RIB EMPLOYEE BEGAN TO MISS WORK ON 01-23-95
EE REPORTED HE STRUCK HIS HARDHAT HEAD AGAINST THE UNDERSIDE OF THE OPERATOR S CANOPY OF A SHUTTLE CAR. THIS OCCURRED WHEN THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROADWAY. EE CONTINUED TO W ORK UNTIL 2-10-95 FIRST DAY MISSED WAS 2-13-95 EE DID NOT INFORM THE OPERATOR THAT THE ACCIDENT WAS THE CAUSE OF HIS ABSENCE FROM WORK UNTIL 3-3-95.
EMPLOYEE WAS OPERATING A JOY 21SC SHUTTLE CAR. AS THE SHUTTLE CAR WAS TRAMMING UNDER THE BOOM OF THE CONTINOUS MINER, THE CONTACTOR TIPS BURNED TOGETHER & THE SHUTTLE CAR WENT INTO HIGH TRAM C AUSING IT TO STRIKE THE REAR BUMPER OF THE CONTINUOUS MINER. THE EMPLOYEE REPORTED PAIN IN HIS NECK AS A RESULT OF THE COLLISION.
EE WAS ATTEMPTING TO PUT A PIN IN THE SCOOP BUCKET JACK. EE PULLED HYD LEVER IN ACTIVATING JACK, IT CAUGHT EE'S HAND BETWEEN THE JACK AND SCOOP BUCKET.
EMPLOYEE WAS OPERATING A 48 VOLT, BATTERY POWERED DAMASCUS 4 WHEELER ON A GRADE IN THE #2 ENTRY. THE CIRCUIT BREAKER TRIPPED AND THE 4 WHEELER (RUBBER TIRED) BEGAN TO ROLL DOWN THE GRADE. THE UNIT STRUCK THE RIB AND THE EMPLOYEE REPORTED AN INJURY TO HIS BACK. HE SAID HE WAS BOUNCED AGAINST THE REAR FENDER OF THE 4 WHEELER.
EE AND HIS CREW MEMBERS WERE PASSENGERS ON A S & S BATTERY POWERED SCOOP ENROUTE TO THE WORKING SECTION AT THE BEGINNING OF THE SHIFT. ON A GRADE, A PIN CAME OUT OF THE BRAKE PEDAL ON THE SCOO P AND THE SCOOP STRUCK A DEPRESSION IN THE ROADWWAY. EE REPORTED HE WAS JOLTED AND EXPERIENCED PAIN IN HIS LOWER BACK.
EMPLOYEE WAS OPERATING A JOY 10SC SHUTTLE CAR TRAVELING EMPTY FROM THE FEEDER TO THE FACE AREA SHUTLE CAR STRUCK A DEPRESSION IN THE ROADWAY & JOLTED THE EMPLOYEE AGAINST THE UNDERSIDE OF THE CANOPY HE EXPERIENCED PAIN IN HIS NECK EMPLOYEEE DID NOT REPORT TO WORK ON HIS NEXT REGULAR SCHEDULED SHIFT EMPLOYEE HAS A HISTORY OF NECK PROBLEMS FROM A PREVIOUS INJURY
EE WAS OPERATING A JOY 21SC SHUTTLE CAR ON THE WORKING SECTION. EE STATED THE SHUTTLE CAR STRUCK A DEPRESSION IN THE ROAD WAY NEAR WHERE THE TRAILING CABLE WAS ANCHORED THIS RESULTED IN THE EE BEING JOLTED IN THE OPERATOR S SEAT. EE REPORTED A PAIN IN HIS LOWER BACK AS A RESULT OF BEING JOLTED.
OPERATING EMPTY SHUTTLE CAR RETURNING TO MINER. EMPLOYEE STRUCK ROCK IN ROADWAY AND MOMENTARILY LOST CONTROL OF THE SHUTTLE CAR. EMPLOYEE WAS BOUNCED AROUND INSIDE THE OPERATOR COMPARTMENT. HE STRUCK HIS TAILBONE AGAINST SEAT AND INJUREDHIS LOWER BACK.

INDIVIDUAL WAS OPERATING FOUR WHEELER, HIT ROCK IN ROADWAY JERKING STEERING WHEEL OUT OF HIS HAND. THE STEERING KNOB HIT AND INJURED LEFT FOREARM.

PERSON WAS TRAMMING SCOOP IN NO 5 ENTRY WHEN CANOPY RUBBED THE ROOF & THE CANOPY BROKE LOOSE FROM THE LEGS ON THE SCOOP & STRUCK THE LEFT LEG CAUSING INJURIES
MANBUS HE WAS RIDING IN WAS DRIVEN THROUGH A DOOR.

WHILE TRAVELING DOWN TRACK ENTRY IN A MANBUS,HE RAISED HIS HEAD AND STRUCK THE MINE ROOF.
PUTTING A POLE ON MANBUS, HIS FOOT HIT THE OCNTROLLER CAUSING THE MANBUS TO MOVE FORWARD, ROLLING HIM OFF THE TOP AND CAUSING HIM TO FALL.
RIDING MANTRIP OUTSIDE AND THE MANTRIP LOST TRACTION. WHEN IT BEGAN TO SLIDE BACK. A SECOND MANTRIP STARTED OVER THE SLOPE AND THEY BUMPED INTO EACH OTHER CAUSING HIM TO HIT HIS LEG.
HE WAS TURNING AROUND ON THE SHUTTLE CAR & SLIPPED TWISTING HIS KNEE.
SPLICING BELT THE BELT STARTED RUNNING AND HE MOVED DOWN AND ROLLED OFF HITTING THE BELT STRUCTURE
I.NAME MISSED WORK ON 5-13-88 AND 5 16-88 CALLED TO SAY HE HAD BEEN INJURED ON JOB 5-07-88 WHILE LOADING ROCKDUST.EOMPANY WAS PROTEDSTING THIS INJURY BUT ON 6-22-88 WE WITHDREW OUR PROTEST.
OPERATING SHUTTLE CAR & HIT DIP OR DROP OFF CAUSING SEVERE PAIN IN BACK.
HE WAS RIDING I3 A SCOOP AND THE SCOOP RAN OVER A ROCK CAUSING HIM TO HIT HIS HEAD AGAINST THE TOP OF THE CANOPY.
HE WAS BEING PU8HED OVER A HILL (HIS SCOOP HAD NO POWER) AND AS THE SCOOP CAME OVER THE HILL, IT HIT A HOLE BOUNCING HIM & CAUSING HIM TO HIT HIS FOREHEAD ON THE CANOPY.
