I NAME WAS OPER5TING A SCOOP WHILE SETTING A BELTHEAD,HE WAS PUSHING THE BELT HEAD WITH THE DECK END OF THE SCOOP.THIS IS AGAINST COMPANY POLICY.I MANE SAID HE FORGET WHICH WAY HE HAD HIS DIRC DIRECTION SWITC2 TURNED AND I NAME DECK INTO THE BELT HEAD THROWING HIS SHOULDER INTO THE TOP AND A DISLOCATION OCCURRED.
