EMPLOYEE DROVE PICKUP TRUCK NO. 1264 TO LOWER GATE TO LOCK SAME. HE DROVE THRU THE OPEN GATE AND TURNED AROUND AND DROVE BACK THRU THE GATE FACING UPGRADE. HE STOPPED THE TRUCK APPROX. 20' AHE AD OF THE GATE, SET THE PARKING BRAKE, EXITED THE TRUCK AND WALKED DOWN THE GRADE. HE PULLED BOTH SIDES OF THE GATE SHUT, CLOSED AND LOCKED THE CHAIN AND AT THAT TIME HE WAS STRUCK BY THE PIC

EMPLOYEE WAS DR3VING AMONIA NITRATE TRUCK TO BLASTING AREA UP GRADE AND WHEN ALMOST AT THE TOP HE LOST POWER AND ATTEMPTED TO SHIFT INTO A LOWER GEAR AND THE TRUCK STARTED BACKWARDS DOWN THE S LOPE. ROAD WAS3DRY WITH LOOSE MATERIAL AND BRAKES WOULD NOT HOLD. HE MADE A SHARP TURN AND OVERTURNED.
