THE EMPLOYEE WAS OPERATING HIS MOTOR, BRING IN A SUPPLY TRIP. THEY CROSSED OVER A DEADBLOCK ON THE WIRE AND LOST POWER AND WERE UNABLE TO CONTROL THEIR TRIP. THE EMPLOYEE GOT DOWN IN THE KITCH EN OF THE MOTOR AND HIT HIS KNEE ON THE DEADMAN SWITCH BEFORE THE TRIP CAME OT A STOP.

EMPLOYEE WAS RIDING IN A JEEP GOING TO HIS ASSIGNED WORK AREA: IN THE PROCESS THE EMPLOYEE THOUGHT A BELT SPLICING MACHINE FELL OFF THE BACK OF THE JEEP. THE EMPLOYEE RAISED UP IN THE JEEP TO CHECK AND HIT HIS HEAD ON A BEAM.
EMPLOYEE WAS OPERATING A SHUTTLE CAR IN AN AREA CLOSE TO THE FEEDER WHICH WAS BEAMED WITH CRIBS UNDER THE BEAMS THE OPERATOR HIT INTO THE CRIBS DISLODING THEM AND ONE CAME INTO THE KITCHEN HIT OF THE SHUTTLE CAR AND HIT HIM IN THE RIB AREA EMPLOYEE SUFFERED A BROKEN RIB EMPLOYEE HAS BYRS EXPERIENCE ON SHUTTLE CAR.

THE INJURED MAN WAS LYING ON THE MINE FLOOR, ATTEMPTING TO LOCK THE PANL INE SECTIONS TO1ETHER & HAD INADVERTENTLY PLACE HIS LEFT FOOT UNDER THE RAISED SECTION OF PAN. BECAUSE THE "DOG-BONE" PIN WAS ON A BIND, ANOTHER MAN MOVED THE PANLINE SECTION @ THE SCOOP CAR-
EMP WAS OPERATI3G A LOCOMOTIVE WHEN THE TROLLEY POLE JUMPED OFF THE WIRE AND BROKE HE A TTEMPTED TO STOP THE MOTOR BUT COULD NOT CLAIMING THE BRAKE WAS INOPERATIVE HE TRIED TO STOP THE MOTOR A AND WHEN HE OBS9RVED A DERAIL ,DEAD LATCH, AHEAD HE JUMPED OFF NOTE-INVESTIGATION REVEALED THE BRAKE WAS OPERATIONAL EMP MUST HAVE PANICKED AND WOUND BRAKE OPEN(AS FOUND) RATHER THAN CLOSED
JEEP POLE CAME OFF THE TROLLEY WIRE WHILE MOVING, INJURED REACHED UP TO RETRIEVE POLE AND STRUCK HIS HEAD AGAINST TROLLEY WIRE GUARD BOARDS. RESULTING IN A CERVICAL AND TRAPEZIUS STRAIN.
INJURED WAS TURING THE PLE ONT HE ROCKDUSTERS MOTOR & FELT A SHARP PAIN IN HIS RT ELBOW. INJURED DID NOT REQUEST MEDICAL ATTENTION & CONTINUED TO WORK UNTIL 10-12-84. ON THIS DAY HE COMPLAINE D OF PAIN & WAS SENT FOR X-RAYS. THE FRACTURE WAS DETECTED & THE EE WAS PLACED ON INJURED STATUS AS OF 10-15-84.
BELT STARTED UP -HE GAVE SIGNAL TO START-TAKE UP CARRIAGE MOVED BACK RUNNING OVER HIS RIGHT FOOT THAT WAS POSITIONED ON RAIL. HAS FRACTURES OF 1ST 2ND & 3RD TOES REQUIRING SUTURES.
RIDING IN PORTAL BUS AT QUITTING TIME. THE INJURED HAD HER RIGHT HAND ON BAR INSIDE OF BUS. THE DRIVER HAD TO OPEN THE OVERHEAD WINDOW TO HANDLE THE TROLLEY POLE. IN DOING SO THE WINDOW PINCHE D RIGHT MIDDLE FINGER CAUSING LACERATION & FRACTURE TO DISTAL END OF FINGER.
OPERATING A HAULAGE MOTOR THE POLE CAME OFF HE REACHED UP TO CATCH IT STRUCK HIS LEFT THUMB ON POLE FRACTURE TO THUMB
RIDING ON HAULAGE IN OPEN JEEP STRUCK HIS HEAD ON LOW ROOF. HAS CERVICAL PAIN IN NECK AREA. INDIVIDUAL HAD BEEN WARNED OF LOW ROOF AREA.
HOOKING UP COUPLER TO MOTOR GOT INDEX FINGER ON LEFT HAND PINCHED BETWEEN COUPLER AND MOTOR REQUIRED .
