INJURED WAS TRAMMING A SHUTTLE CAR ON A ROUGH ROADWAY WHICH CAUSED HIS HEAD TO STRIKE THE CANOPY
INJURED COMPLAINED OF BACK PAIN WHICH HAD AGGRAUATED HIM ALL WEEK UNTIL HE COULD NOT OPERATE SHUTTEL CAR. HE FELT THE PAIN WAS DUE TO BEING JOSTLED AROUND IN THE SHUTTLE CAR.
INJURED WAS OPERATING A S-TONG MOTOR TRAVELING ON 55 FACE NORTH HAULAGE. APPROUCHING 57 MAINS EAST HAULAGE, A MOTORMAN WITH TWO 10-TONE MOTORS WAS COMMING THROUGH SWITCH OFF OF 57 MAINS EAST HAUL BE AND ALL 3 MOTORS CAME TOGETHER. THE 5-TLON MOTOR WAS COMING TOO FAST TO STOPE ADN INJURED WAS THROWN FORWARD INTO CANOPY.
INJURED WAS STANDING IN FRON TOF BUCKE TOF SCOOP DIRECTING THE UNLOADING OF SOLID BLOCK FOR A STOPPING. INJURED INSTRUCTED OPERATOR TO TRAM OUTBY BUT BEWARE DOING SO, OPERATER SWUNG BUCKET W HICH WAS A COUPLE OF INCHES OFF THE GROUND. AS HE DID SO THE BUCKET GRABBED ACROSS INJURED'S RIGHT FOOT.

INJURED WAS RIDING MANTRIP AS IT WAS LEAVING SECTION AT QUITTING TIME. AS IT CAME TO SECTION SWITCH BLOCK LIGHT WAS TRIPPED FOR RIGHT-OF-WAY. A MANTRIP FROM ANOTHER SECTION WAS APPROACHING AND COULD NOT SLOW FOR THE INJURED'S MANTRIP IN ENOUGH TIME, RESULTING IN A COLLISION, STRAINING INJURED'S BACK.

TENSION ON ROPE CAUSED INJURED TO LOSE HIS GRIP AND HE WAS STRUCK BY WHIPPING E
INJURED WAS OPENING A 20 TON LOCOMOTIVE WITH A TRIP OF EMPTIES ENROUTE TO MOTORBARN. HE WAS CHECKING THE ELECTRIC BUNDLE ON THE MOTOR & LOOKED @ AMP CAUGE TO MAKE SURE WAS WORKING. WHEN HE T URNED HIS HEAD BACK TO LOOK IN DIRECTION OF SHOVEL HE HIT HARD HAT AGAINST OVERHEAD BEAM.
EMPLOYEE WAS RIDING IN THE INBY PASSENGER SEAT OF A 5-TON BATTERY BUS WHEN THE OPERATOR FAILED TO SEE TWO TRUCKS PARKED ON THE TRACK AND THEIR BUS COLLIDED WITH THE TRUCKS.
EMPLOYEE WAS TIGHTENING A BOLT FOR A CHUTE ABOVE A CONVEYOR BELT WITH A LONG HANDLED RATCHET WRWENCH. TEH HANDLE OF THE WRENCH CAME IN CONTACT WITHT HE TOP CONVEYOR BELT AND WAS FACING UPWARD , CAUSING HIS LEFT HAND TO BE CAUGHT BETWEEN THE END OF THE RATCHET AND CHUTE.
INJURED WAS LOADING SHUTTLE CAR BEHIND CONTINUOUS MINER THE MINER OPERATOR BACKED UP AND HIT SHUTTLE CAR WHICH CAUSED A SUDDEN JERK IN SUDDENT IMPACT.
EMPLOYEE PUSHED JEEP ON TRACK AND GOT OUT TO REPAIR GUARD BOARD. A MOTOR WAS APPROACHING AND MOTOR TROLLEY POLE CAME OFF DC WIRE. INJURED WAS GETTING OUT OF JEEP WHEN MOTOR COLLIDED WITH JEEP. INJURED FELL AND LANDED ON SELF-RESCUER ON HIS LOWER BACK.
INJURED'S RIGHT FOOT WAS CAUGHT BETWEEN A CRIB BLOCK AND A SHIELD SKID WHEN THE SCOOP PUSHED THE CRIB BLOCK BACKWARDS.
EMPLOYEE WAS LEAVING 3 BOTTOM AS A PASSENGER IN A 5TON MOTOR AS THE MOTOR CAME INTO THE PANALL SWITCH AT #3 BOTTOM THE OPERATOR COULD NOT STOP & RAMMED INTO ANOTHER 5 TON MOTOR
INJURED WAS INSTRUCTING THE CHELOADING OF A LOAD CENTER AND HAD PLACED HIS LEFT HAND ON SHOVEL ON SCOOP BETWEEN BLADE OF BUCKET & BACK SIDE OF BUCKET. INJURED NSTRUCED OPERATOR TO LIFT BUCKET AND HIS FINGER WAS CAUGHT BETWEEN BUCKET & FRAME OF SCOOP.
EMPLOYEE WAS OPERA TING EIMCO SCOOP WHEN THE DOOR CAME OPEN ON THE CAB EMPLOYEE REACHED OUT WITH HIS LEFT HAND & PULLED THE DOOR CLOSED WHILE THE SCOOP WAS STILL MOVING & CAUGHT HIS FINGERS BE TWEEN THE DOOR & STEERING JACK
EMPLOYEE WAS STANDING ON THE INBY SIDE OF SCOOP WHICH WAS BACKING OVER RAILS AS SCOOP BACKED UP BOTTOM OF SCOOP BUCKET CAUGHT ON A RAIL PUSHING IT TOWARD INJURED.
INJURED WAS CHECKING SHUTTLE CAR CABLE WHEN THE SHUTTLE CAR WAS PUSHED BACKWARDS AND CONTACTED HIM.

54736YEE LEFT MOTOR BARN AFTER HAVING #1 MOTOR REPAIRED. DUMPER 1810 MOTORMAN TO TAKE AWAY 18 WAGONS. TRAIL MOTORMAN CAME TO COUPLE UP TO EMPLOYEE IN LEAD MOTOR, BUT GOT NO RESPONSE. TRAIL 46369MAN FOUND LEAD MOTORMAN UNCONCIOUS AND LYING FACE DOWN ON MINE FLOOR.
EMPLOYEE WENT TO TAKE A 10-TON MOTOR OFF CHANGE AT #3 AIR SHAFT WHEN HE DUCKED DUE TO LOW HANGING FIRE PROOF CLOTH AND FELT PAIN IN HIS LOWER BACK.
UNLOADING ROCK DUST FROM SCOOP,SCOOP RAN OVER R FOOT
EMPLOYEE WAS IN CAB OF CONTINUOUS MINER AND STARTED CONVEYOR TO LOAD COAL. AS COAL WAS BEING LOADED, A PIECE OF SLATE APPROX. 4"X18' RODE THE EDGE OF THE CONVEYOR AND STRUCK OPERATOR WHILE ON THE CAB.
EMPLOYEE WAS IN THE PROCESS OF INSTALLING A TRACK BOLT ON AN ELECTRIC SWITCH. IT WAS DECIDED TO TACK WELD THE BOLT. EMPLOYEE HAD LEFT HAND ON THE TRACK AND WELDER MOVED TOWARD THE SWITCH. THE SWITCH TRIPPED AND CAUGHT EMPLOYEE'S LAST TWO FINGERS OF HIS LEFT HAND.
INJURED WAS STANDING BETWEEN 2 ROLLS OF BELT AND SCOOP BUCKET HIT ONE ROLL CAUSING IT TO TRAP INJURED BETWEEN BOTH ROLLS OF BELT.
EMPLOYEE WAS RIDING IN A PERSONAL CARRIER WHEN IT DERAILED AND STRUCK A COAL RIB CAUSING HIM TO BE THROWN FORWARD STRIKING HIS HEAD ON THE PORTABUS RESULTING IN A 2" LACERATION TO THE BACK OF HIS HEAD.
WHILE OPERATING AN 8 TON LOCOMOTIVE THE TROLLEY POLE DISLODGED FROM THE WIRE.WHEN HE TURNED TO SEE THE POLE HIS HEAD STRUCK A LOW HWADER BLOCK CAUSING HIS GLASSES TO BE PUSHED DOWN ON HIS NOSE RESULTING IN A LACERATION TO HIS NOSE AND NECK PAIN.
WITH THE BELT RUNNING THE INJURED ATTMEPTED TO RE-INSERT A BELT FLAP USING A SLATE BAR, WHEN THE SLAT BAR CAUGHT IN THE BELT THEN STRUCK HIM ON THE BACK OF THE HEAD CAUSING HIM TO FALL TO MINE FLOOR.
WHILE RIDING AS A PASSANGER IN A 5 TON MANTRIP OPERATORS SEAT THE MANTRIP STRUCK A LOWBOY PARKED ON THE LHAULAGE CAUSING THE INJURED TO STRIKE THE BACK OF HIS HEAD ON THE CANOPY OF THE MANTRIP RECIEVING A LACERATION TO HEAD AND BACK PAIN.
INJURED AND 2 CO-WORKERS WERE SHOVELING SPILLAGE AROUND BELT TRANSFER WHEN A FIRE STAND COUPLER BROKE APART. THE INJURED WAS STRUCK INT HE FACE AND NOSE WITH A COUPLER ATTACHED TO THE HOSE RE SULTING IN A FX NOSE.
INJURED AND 2 CO-WORKERS WERE SHOVELING SPILLAGE AROUND BELT TRANSFER WHEN A FIRE STAND COUPLER BROKE APART. HIGH PRESSURE WATER FORCED HIS LEFT LEG TO CONTACT THE BELT.
EMPLOYEE PUT OUTRIGGERS DOWN.TWON ON PAVED ROAD AND TWO ONTO GRAVEL ALONG ROAD.EMPLOYEE BOOMED OUT TO THE LEFT SIDE OF TRUCK APPROXIMATELY 30 FT.THE DOWN RIGGERS SUNK INTO THE GRAVEL AND THE T RUCK TURNED OVER.JUST BEFORE THE BUCKET HIT THE GROUND,THE EMPLOYEE JUMPED OUT AND BUCKET STRUCK EMPLOYEE.
EMPLOYEE WAS RIDING IN A BELT PERSONEL CARRIER BEING TOWED BY A MOTOR WHEN THE COUPLERS ON THE 2 VEHICLES MISALIGHNED. THE MOTOR STOPPED CAUSING THE JEEP TO STRIKE THE MOTOR RESTING IN A CERVI CAL STRAIN TO THE INJURED.
INJURED WAS RIDING IN A 20 TON LOCOMOTIVE WHEN HE REACHED FOR THE BRAKE HANDLE HE RAISED HIS HEAD AND STRUCK IT ON A LOW BEAM.

INJURED WHILE OPERATING A SCOOP RAN OVER A ROCK IN THE ROADWAY CAUSING HIS HEAD TO HIT THE OPERATOR'S COMPARTMENT CANOPY.
INJURED WAS OPERATING A 10TON LOCOMOTIVE WHEN HE STRUCK A 5 TON LOCOMOTIVE COMING OUT OF A SWITCH CAUSING HIM TO STRIKE HIS BACK RT KNEE & HIP ON THE CONTROL BOX INSIDE THE LOCOMOTIVE CAB
EMPLOYEE WAS OPERATING A 5 TON MANTRIP. WHEN THE BRAKES LOCKED UP CAUSING HIM TO SLID INTO A PARKED FLAT CAR. THROUGHLY HIM FORWARD STRIKING HIS EAR ON THE CANOPY OF THE MANTRIP. RESULTING IN A LACERATION TO HIS LEFT EAR.
