SHOVELING FINES ONTO BELT. AS HE SLID SHOVEL UNDER BELT IT HIT A ROCK AND SHOVEL CONTACTED A TURNING RETURN ROLLER. THE SHOVEL HANDLE KICKED BACK AND STRUCK THE INJURED'S RIGHT KNEE.
WAS HANGING A VENT TUBE AT THE FACE. A SHUTTLE CAR'S AUTOMATIC FIRE SUPPRESSION DISCHARGE AND THE DRY CHEMICAL POWDER BECAME AIRBORNE. THE EE INHALED SOME OF THE POWDER AND CLAIMED THAT HIS LU NGS WERE BOTHERING HIM.
EE WAS OPERATING SCOOP HE HAD HIS HEAD TURNED IN THE DRIVING DIRECTION. A ROCK FELL ON THE BACK OF THE SCOOP AND HE TURNED HIS HEAD. AFTER THER THE TURN OF THE HEAD HIS NECK BEGAN TO HURT
WAS OEPRATING A SHUTTLE CAR WHEN HE HIT A BUMP IN TRAM ROAD AND JARRED HIS BACK.
WAS OPERATING PORTO-BUS TRAVELING INTO MINE. HE REACHED OVER HEAD TO TURN ON A SIGNAL LIGHT BLOCK SWITCH AND FELT A PAIN IN HIS SHOULDER. HE HAS A HISTORY OF NECK AND SHOULDER PROBLEMS WHICH M AY HAVE CONTRIBUTED TO THIS ACCIDENT.
CREW WAS MUCKING. WHEN LEAD MINER BELLED BUCKET OFF OF STEADY AND AT THE SAME TIME INJURED BENT OVER TO PIC UP A PIECE OF CHALK AND WHEN HE RAISED UP, HIS HEAD HIT THE BUCKET.
WAS IN MANTRIP. WHEN HE GOT OUT HE FELT A PAIN IN BACK. EE DID NO WORK OR ACTIVITY.
WAS OPERATING A LOADING MAHCINE WHEN THE SHUTTLECAR RAN INTO THE BACK OF THE LOADER JARRING THE OPERATOR OF THE TRACTOR. APPARENTLY HE SUSTAINED A ROTATOR CUFF TEAR. HE CONT TO WORK UNTIL 4/8/ 98 WHEN HE WAS DISABLED BY A DR AND SCHEDULED FOR SURGERY
EE WAS STANDING ON LOCKED OUT SHIELD. THE PAN WAS BEING PULLED BACK DUE TO A STUCK FACE CONVEYORWHEN THE PAN LINE WAS RETRACTED THE PIN TO THE RELAY BAR BROKE AND THE E'S LEFT HAND WAS BRUISED WHEN IT HIT THE MINE FLOOR.
WAS OPERATING LOCOMOTIVE AND TURNED TO LOOK BACK. WHEN HE TURNED FORWARD HE HIT HIS HEAD ON A BEAM.
A JEEP OPERATOR WAS TRYING TO PUT JEEP ON TRACK BY PULLING THROUGH A TRACK SWITCH. THE WHEEL WHICH WAS OFF CLIMBER OVER THE RAIL. THE VEHICLE HIT EE IN THE LEG. EE WAS IN A KNEELING POSITION W ATCHING THE WHEEL TO CHECK TO SEE IT IT WENT BACK ON TRACK.
WAS WALKING FROM FACE WHEN A SHUTTLE CAR STARTED. THE SHUTTLE CAR CABLE PICKED UP AND HIT HIM ON THE ANKLE KNOCKING HIM DOWN.
EMPLOYEE HAD LOADED TUBES IN SHUTTLLE CAR WHEN WSHUTTLE CAR STARTED EMPLOYEE HAD MOVED NEAR CABLE SNUB. THE CABLE WHIPPED UP AND STRUCK HIM ON FACE CUTTING HIS LIP.
INJURED WAS USING SLOPE HOSIT CAR TO POSITION TWO SUPPLY CARS. A SKID HAD BEEN LEFT UNDER THE LEAD CAR. WHEN THE SECOND CAR RAN OVER THE SKID , THE TRIP WAS STOPPED. WHEN THE INJURED REACHE D FOR SKID THE CARS MOVED AND HIS FINGER WAS CAUGHT BETWEEN THE CAR WHEEL AND SKID.
WAS OPERATING A LOCOMOTIVE PULLING TWO CARS OF SUPPLIES. THE MOTOR SPLIT THE SWITCH AND DERAILED. THE EE REPORTED ON 9/23/96 THAT HIS BACK HURT AND BECAME A LOST TIME ACCIDENT.
INJURED WAS KNOCKING OUT A VENTILATION MASONRY WALL WITH A BATTERY SCOP. THE SCOOP GOT HUNG UP AND IN TRYING TO FREE SCOOP SOME MAKERIAL BUMP PART OF WALL ON THE SIDE WHICH WAS STILL STANDING. A PIECE OF BLOUL 9"X5"X5" FELL FROM THE SIDE AND STUCK INJUREDS LEFT HAND WHICH WAS RESTING ON SCOOP FRAME.
WAS BENT OVER GREASING A ROOF DRILL WHEN A SHUTTLE CAR HIT VENTILATION TUBES KNOCKING TUBES LOOSE. A TIC WIRE CUT EMPLOYEE ON THE LEFT EAR.
WAS OPERATING A LOCOMOTIVE. HIS SCSR WAS ON TOP OF THE MOTOR AND BEGAN TO VIBRATE OFF THE MOTOR. HE REACHED FOR HIS SCSR AND HIS HEAD HIT THE EDGE OF AN OVERCAST BEAM.
WAS OPERATING SHUTTLE CAR WHEN SHUTTLE CAR CANOPY ROOFED OUT. THE CANOPY WAS TORN OFF AND THE SHUTTLE CAR OPERATOR WAS JOLTED AND SUFFERED A STRAINED NECK AND BACK. HE WORKED ON 1-1-96. HE SAW A DOCTOR ON 1-16-96 AND WAS DISABLED.
EE WAS OPERATING A MOTOR AND PULLING IT INTO A SWITCH TO PUT IT IN CHARGE. THE MOTOR JUMPED TRACK--JOLTING EE AROUND IN THE KITCHEN. SPRAINED HIS NECK.
WAS RIDING IN MANTRIP. MANTRIP ATTEMPTED TO STOPAND SLID INTO ANOTHER MANTRIP. INJURED BRACED HIMSELF AND HIS HAND WAS CAUGHT. WHEN THE BATTERY CASE SLID INTO SEAT BACK.
STANDING WAITING FOR SHUTTLE CAR, CABLE OF SC HIT LOWER LEGS AND FLIPPED HIM. HE INJURED HIS SHOLDER WHEN HE HIT THE GROUND.

TRAMMED THE 630 PAN SCOOP IN THE A PANEL BELT LINE. RAN OVER A BENT BEDRAIL, BEDRAIL CAM EUP AND HIT EMPLOYEES MINE HAT FIRST DAY OFF WORK 11-10-93.

INJURED WAS STANDING IN A CROSSCUT OBSERVING A LOCOMOTIVE PULL A LOWBOY INTO THE AREA. WHEN THE MOTOR PASSED, HE STEPPED OUT AND WAS STRUCK BY THE LOWBOY. KNOCKED DOWN AND DRAGGED A SHORT DIST ANCE.
WHILE OPERATING SHUTTLE CAR INJURED STATED THAT THE AUTOMATIC BRAKES LOCKED UP AND THE SUDDEN STOP CAUSED HIM TO HIT HIS HEAD ON THE CANOPY.
INJURED WAS PULLING ROCKDUST TANKS WITH A LOCOMOTIVE OUT OF A PANEL. HE LOST CONTROL OF VEHICLE. THE TRIP CAME TO A SUDDEN STOP WHEN THE TRAILING LOCOMOTIVE DERAILED. THE SUDDEN STOP RESULTING IN THE INJURED HITTING HIS KNEES AGAINST THE FRAME.
INJURED WAS TRAMMING SCOOP LOADED WITH BLOCK. HE STRUCK RAIL LAYING ON GROUND WHICH STRUCK A TIE STANDING ON THE RIB. THE TIE FEEL STRIKING INJURED ON RIGHT EAR AND SHOULDER.

INJ AND COWORKING STANDING ON TAIL OF #1 HAULAGE 6 BELT CHANGING BELT WIPER ON #1 HAULAGE 7 BELT WHEN 6 BELT STARTED.THEY WERE THROWN INTO /UNDER BELT CHUTE.EMPLS HAD FAILED TO LOCK OUT 6 BELT .
THE INJURED AND CO-WORKER WERE STANDING ON THE TAIL OF THE #1 HAULACE 6 BELT CHANGING A BELT WIPER ON #1 HAULAGE 7 BELT. WHEN THE #1 HAULAGE 6 BELT STARTED, THEY WERE THROWN INTO/UNDER THE BE LT CHUTE. THE EMPLOYEES HAD FAILED TO LOCK-OUT THE #1 HAULACE 6 BELT.
