A ROLL OF BELT WAS CHAINED TO THE 930 LOADER BUCKET. THE CHAIN CAME UNHOOKED AND THE BELT FELL FROM THE LOADER PUSHING INJURED MAN BACK.
WHILE OPERATING SHUTTLE CAR EE HIT A ROUGH SPOT CAUSING HIM TO BOUNCE UP AND AS HE FEEL DOWN ON SEAT HE HURT HIS RIGHT TESTICLE.
SHUTTLE CAR CAUGHT MINER CABLE ON BUMPER AND AS SLACK STRIGHTENED OUT AND STRUCK EMPLOYEE ON LEFT ANKLE CAUSING AN OPEN WOUND AND FRACTURE.
AFTER EMPLOYEE MAN TRIPPED FROM THE SURFACE TO THE SECTION APP 10 MIN TRIP HE STATED THATH E WAS EXPERIENCING PAIN IN HIW LOWER BACK THIS EMPLOYEE HAS A PAST HISTORY OF BACK PROBLEM NO ACCIDEN REPORTED.
EMPLOYEE BEGAN 7OMPLAINING OF SEDVERE B ACK PAIN & MUSCLE SPASMS WHILE DRIVING 777 CAT TRUCK EMPLOYEE HAS ALSO MENTIONED HAVING SIMILAR BACK PAIN BEFORE COMING TO WORK THIS INCIDENT STILL PEND ING DOC. REPORT3
