THE EMPLOYEE WAS CLEANING THE BELT TAIL ROLLER UNDER THE BIN WITH A PIECE OF WOOD WITH THE BELT OPERATING. THE LACES ON THE BELT CAUGHT THE EMPLOYEE'S GLOVE AND DRUG HIS ARM AROUND THE ROLLER INTO THE BIN. THE FRONT END OPERATOR SAW THE EMPLOYEE WHEN HE WAS DUMPING A BUCKET OF MATERIAL INTO THE BIN AND TURNED THE BELT OFF. THE GUARD OVER THE BELT ROLLER ASSEMBLY HAD BEEN REMOVED AN
