EE was assisting a co-worker to attach a belt tool trailer to a battery operated ride at break No. 41 on No. 7 roadway. While co-worker was moving the ride, EE was struck by the ride resulting in contusion of both legs.

EE was riding 4-wheeler up the slope, battery went dead, stopped to let battery re-energize, brakes failed, tried to steer ride into rib, ride rolled over on him. Did not miss work until 7/27/10.

EE was riding in a mantrip that was pulling a broken down mantrip. He was sitting on his side when the chain pulled tight. He ALLEDGEDLY hit his hip on a piece of metal on the back of the seat. This injury was not approved by Brick Street that occurred in the course of and as a result of covered employment until August 13, 2010.

EE was hauling a load of belt structure to the section. The Red Devil cable being used to hold the structure in the scoop bucket broke and struck him in the forehead.

Employee was walking by #2 Schuttle Car anchor, cable was taking up and the anchor hit him in the face and he received a cut above his left eye.

Day shift electrician was assisting in moving a continuous miner underground. An electrical power move box was being transported in the bucket of a scoop. His right middle finger was cut/crushed when box was being pushed out of scoop bucket.

EE "ALLEGED" that while operating a shuttle car, he hit a hole in the roadway causing a back injury. His first day absent from work was 8/2/06.

Employee was backing up the mantrip when the steering wheel spun around and the steering knob struck him on the left thumb.

Employee was standing on shuttle car cable, cable took up slack and flipped him backward and hit his head on mine bottom.

ALLEGED to have hit a hole with a coal hauler and hit head on canopy injurying head and neck. He continued working that shift with 5 hours overetime and the 3 additional scheduled days that week. He did not report the injury until 11/01/04 via phone to the buisness manager. His first scheduled workday missed was 11/03/04. This injury is being contested to Workers Comp.

Employee injured back while using Coal Hauler to bump Continuous Miner away from rib, so it could be repaired.

Employee was struck in the face when a flyboard entered the operators compartment of his shuttle car one break outby the face. His first day missed work was 4/21/04

EMPLOYEE STATED HIS RIGHT MIDDLE & INDEX FINGERS WERE CAUGHT BETWEEN THE OPERATOR'S COMPARTMENT FRAME & THE TRAILER FRAME OF THE #357 BATTERY COAL HAULER WHEN STEERING RIGHT HANDED.

EE WAS HOOKING UP CHAIN TO MOVE POWER CENTER. ANOTHER EE WAS OPERATING SCOOP. FLOOR OF MINE WAS ON SLIGHT INCLINE AND SCOOP WAS COASTING BACK TOWARDS POWER CENTER. EE WAS TO SIDE OF POWER CENT ER. SCOOP STOPPED ROLLING. EE STEPPED INTO HOOK CHAIN. SCOOP JUMPED IN TRAM AND HIT HIS LEG AND KNEE ON RIGHT SIDE.

EVENING SHIFT CONTINUOUS MINER OPERATOR WAS DRAGGING MINER CABLE WITH LONG-AIRDOX THREE WHEELER WHEN PULL ROPE SLIPPED OFF PULL HOOK AND CAUGHT HIS RIGHT ANKLE.

INJURIED LEFT KNEE RUNNING S/CAR ON INCLINE PUSHED BRAKE INJURING LEFT KNEE. INJURED HAD SURGERY ON THIS KNEE DUE TO A OLD ACCIDENT.

EE WAS HAULING COAL IN 10 S/C JOY SHUTTLE CAR WHEN HE APPARENTLY HIT POTHOLE IN MINE FLOOR AND INJURED HIS LOW BACK.

OPERATING SHUTTLE CAR RAN THRU A HOLE, SHUTTLE CAR BOUNCED INJURED LOWER BACK. INJURY BECAME REPORTABLE 3/13/02. THIS WAS THE FIRST MISSED DUE TO INJURY.

EE REPORTED (JAN. 29, 2002) THAT WHILE HAULING COAL ON THUR., JAN. 24, HE HIT A BUMP OR RAN OVER A ROCK IN HAUL ROAD AND INJURED HIS LOW BACK.
