At RNS Barnes/Watkins site on 2/21/07 employee was operating a rock truck when he hit a hole in the haul road. His upper body was twisted in an odd way causing pain to his upper back. He did not know the severity of the injury at the time and did not seek medical attention until 3/01/07.

EE WAS DRIVING TRACTOR TRAILER. THE ROADWAY TO THE REFUSE SITE HAD BEEN WATERED TO SUPPRESS DUST. THE TEMPERATURE HAD DROPPED AND THE ROADWAY FROZE PARTIALLY. EE WAS BLINDED BY BRIGHT SUNLIGHT AND HIS SPEED WAS EXCEEDING THE 10 M.P.H. LIMIT. THE TRUCK FAILED TO NEGOTIATE A TURN AND DROVE OFF THE ROAD, STRIKING A CEMENT WALL.
