I NAME WAS OPER5TING A 651 SCRAPER HAULING OVERBURDEN FROM THE CUT TO THE DUMP. WHILE DUMPING HIS SCRAPER ON THE DUMP AT ABOUT 7 AM HE ENCOUNTERED A ROUGH AREA OF THE DUMP, STRUCK A ROCK OR A HOLE AND THE QU3CK MOVEMENT OF THE SCRAPER CAUSED HIM TO INJURE HIS BACK AND HOP AREA
EMPLOYEE WAS OP9RATING A FORKLIFT. WHILE OPERATING THE FORKLIFT EMPLOYEE PUSHED THE CONTROL LEVER DOWN. WHEN HE PUSHED DOWN THE CONTROL LEVER, HE DISLOCATED HIS RIGHT THUMB. EMPLOYEE ADVISE PERSONNEL THAT3THIS WAS A REINJURY.

LOADER LOADING 2AUL TRUCK WITH COAL.A PIECE OF COAL FELL FROM THE SIDE OF THE LOADER BUCKET.THE COAL HIT THE HAUL TRUCK WINDOW FRAME AND SHATTERED THE WINDOW OF THE HAUL TRUCK.AFTER SHATTERING THE WINDOW GLAS8 WENT INTO THE CAB OF THE TRUCK HITTING THE LEFT SIDE OF THE OPERATOR.AFTER FIRST AID TREATMENT & PHYSICION'S EXAM.SOME SMALL GLASS PARTICLES WERE REMOVED FROM THE OPERATIONS
SEAT BELTS BEIN1 WORE WEATHER CLEAR AND SUNNY. HAUL ROAD, SINVERY GOOD CONDITION OPERATOR WAS COMING OUT OF PIT AROUND THE CURVE AND A WELD THAT HOLDS STEERING PIN IN BROKE. THE PIN FELL OUT AND SCRAPER WE3T OUT OF CONTROL. HE HIT THE BRAKES. AND DROPPED THE BUCKET THE IMPACT CAUSED HIM TO HIT HIS HEAD ON THE SIDE WINDOW OF CAB.
