WHILE ENTERING THE OPERATORS COMPARTMENT, EMPLOYEE TOUCHED THE OPERATORS PEDAL AND THE SCOOP TRAMMED FORWARD CAUSING HIM TO STRIKE HIS ARM AGAINST A ROOF BOLT, CAUSING A SEVERE CUT IN THE WRIS T AREA AND ALSO A BRUISE TO THE ELBOW.

WHILE TRAMMING A 474 S & S SCOOP TO THE SURFACE. THE OPERATOR ENCOUNTERED SOME LOW ROOF CONDITIONS AND HIS HEAD WAS STRUCK BY THE TOP AND A ROOFBOLT PLATE. THE RESULT WAS A SEVERE CUT TO THE SIDE OF THE HEAD.

APPARENTLY HE WAS BETWEEN SCOOP AND RIB WITH WHAT HE THOUGHT TO BE ADEQUATE CLEARANCE. THE SCOOP WAS MOVING SLOWLY BUT SLID SIDEWAYS APPROX. 12-18" AND INTO A RUT IN MINE FLOOR. SCOOP BUMPER S TRUCK END OF HIS STEEL TOE RUBBER WORK BOOT AND CRUSHED IT TO MINE FLOOR. DAMAGE TO 4 TOES WITH 3 BEING SURGICALLY REMOVED AT HOSPITAL LATER IN DAY.
