Employee was in process of installing a keeper on the left side tie rod end on a battery four wheeler, as EE reached up to get EE's channelocks , EE made contact with the foot pedal causing the machine to reverse striking EE's left leg & abdomen, EE continued to work until 9-16-24

EE was watching cathead as it was being moved with scoop, it came unhooked and EE told Co-Worker to back up scoop, when backing scoop up Co-Worker hit EE in both legs resulting in a torn muscle in the left leg below knee.

Tramming scoop in track entry had rt arm arm outside operator's compartment, ran scoop into rib catching arm between scoop and rib causing laceration, resulting in sutures needed to repair the wounds

Employee was helping remove a set of batteries out of battery tray on surface using a lifting beam hooked to the forklift, two other jacks was being used on each side to aid in raising battery out of tray, as they bent over to move the jack the lifting beam came unhooked hitting employee in the face, employee was taken to hospital and diagnosed with sinus cavity fracture

Employee stated employee was cleaning the face and was backing up scoop to set over, employee placed left hand on top of the operator door and got the deck into the rib smashing hand between rib and scoop door causing fractured thumb.

EE was walking to drop cable slack in preparing for power move, an electrician was trouble shooting reel on the #2 car, when the electrician pecked on the valve the cable reel engaged as EE was stepping across cable, it came up striking EE on the mouth area causing laceration to lip requiring couple stitches.

Night shift driver of 036 tractor at Davey Branch Mine was starting the engine on the bed treatment rack. EE's right hand came back and struck the container cage, peeling EE's skin off 3/4 to 1 inch across the top of the hand.

Employee was standing at the dump end of the shuttle car checking why the feeder stalled out, scoop was cleaning the center dump as operator got out of scoop EE hit the PTO lever and it shot oil into EE's left eye behind EE's safety glasses

While tramming feeder from #5 entry around corner into the 5 left crosscut, EE inadvertently contacted rib with one of the tram levers, while having the other tram lever engaged, causing feeder to move towards the rib pinching EE's left hand between rib and feeder, fracturing hand and fingers.

Employee operating #2 shuttle car ran over a rock in #4 entry causing shuttle car to bounce and hit head into the canopy, rock measured 12"x 6" X 6"thick, employee was wearing seat belt. Employee started physical therapy on 7-6-21 which made it made reportable at that time.

Employee was tramming a shuttle car from the miner to the feeder. At spad #13980 there were three hangers stranded together spanning from one bolt to another. One end of the hanger became dislodged swinging down catching the left shoulder of jacket and pulled them from the shuttle car causing injury to left shoulder and right foot.

Employee was riding the outby end of the #9 mantrip when the operator failed to stop at the airlock doors at the 5 head and caught employee's foot against the grating on the mantrip and the door causing 3 broken toes.

The employee and several others were recovering high voltage cable from the 1st left panel freeze fork when EE's foot was caught under the high line sled as it was being pulled by a scoop.

An employee was struck by 4" waterline that came out of scoop bucket causing a fracture to right lower leg. Surgery was required to repair the fracture.

Employee was tramming a supply motor around a curve, the motor derailed causing EE to bump chin on the dashboard of the motor. EE returned to work next scheduled shift and worked until pre-scheduled vacation of 8/21/2019 - 9/03/2019. This office was notified on 10/07/2019 that during this time EE had surgery on 8/21/2019 to correct TMJ, making this a reportable injury.

Rock had fallen from the working face into the conveyor of the continuous miner and was then loaded into the shuttle car. The rock then fell off the shuttle car, striking the employee's left leg, causing fractures to the femur, tibia, and fibula.

While operating a scoop, employee ran over a wooden 4x4 causing neck and lower back to be jammed. Employee then twisted knee getting out of the scoop. Employee was taken to Dr. and received a full release with no restrictions. On 08/29/2017 employer received notes from WC that employee had received physical therapy.

Employee stated that after dumping load of coal on the feeder ee started back toward the miner. About 15 feet from the intersection inby the feeder the shuttle car breaker knocked causing the park brake to engage. The employee was thrown forward and in an attempt to brace self stiffened causing pain in lower back and down legs.

EE was in the process of recovering rail off of the old #4 track entry. They had been using a rope to load the rail onto the rail car. EE told the motor man to pull the motor and rail car outby when the rope got caught on the side post of the rail car causing 2 rails lying on the mine floor to move outby and strike EE behind knee knocking EE to the mine floor.

EE was tramming a 10 S/C end drive shuttle car when he hit a hole in the roadway hurting his back

Employee slipped and grabbed the lever that raises and lowers the drill pot setting the drill pot down on his knee. This injury happened on 07-27-2015 but the employee continued to work until 11-06-2015.

While traveling to section in battery powered ride the breaker knocked and the ride rolled backwards and into the corner of the rib. Hyperextension of the left foot causing fracture to the 2nd metatarsal bone.

They was advancing the track in the track entry and had raised the track rail up with the forklift and the rail puller slipped, allowing the track rail to drop onto his left foot resulting in a hairline fracture to his foot.

Employee was helping roof bolt crew re-route roof bolt cable. As they were moving the cable, a shuttle car came around a turn striking the employee into the rib.

As the shuttle car was going to the feeder, employee was down on his knees hanging another S/C cable when the moving S/C struck employee's legs and feet.

Employee was coming back from the supply hole. As he was turning into a break, he ran over a bucket of degreaser. When the bucket busted, the degreaser went on employee's right arm, shoulder and neck.

Employee was operating a four wheel rubber tire ride when he ran over a piece of rock causing the steering wheel to jerk out of his hand. When the steering wheel jerked, the steering knob struck employee on the top of the right hand.

EE was attempting to cross the 5B belt at the 5B flow-thru tail piece and mistakenly turned belt switch to the "on" position and was carried into the tailpiece. His right foot then became lodged between the skirt board and impact roller, causing a sever laceration to his foot.

Member was riding in a mantrip and started up an incline when they lost traction resulting in the mantrip sliding back into a second mantrip. Employee complained of neck pain.

Employee was going to cont miner with shuttle car. Hit hole in roadway causing his head to hit the canopy.

Member was driving a diesel tractor, the tractor started to bounce, as member hit brake, causing member to hurt right elbow.

Member was operting mantrip, the mantrip jerked. Member hit top of canopy of mantrip hurting his neck.

Member was taking a scoop off charge when he struck his left knee on the side of the scoop.

EE WAS LOADING A BUNDLE OF CRIB BLOCKS ONTO A SCOOP BUCKET FROM FLAT CAR. EE PUT LEFT HAND AT THE BOTTOM OF THE BUNDLE WITH FINGERS UNDER IT IN ORDER TO POSITION IT IN THE SCOOP BUCKET. THE SCOOP OPERATOR RAMMED THE EJECTOR OUT SMASHING FINGERS.

While traveling outside, coming down grade mantrip struck a belt hold down roller that the outby crew was moving into place, roller caught mantrip causing it to come into operator deck hitting him on left side of hip, causing bruise.

EMPLOYEE WAS RIDING A MANTRIP TRAVELING OUTBY ON THE #5HG. A MOTOR LEFT THE BOXCUT CALLING FOR THE ROAD AT DESIGNATED LOCATIONS HEADING TOWARDS #5HG. THE MOTOR RECEIVED THE ROAD TO TRAVEL INBY ON #5HG WHEN IT STRUCK THE MANTRIP TRAVELING OUTBY.

EMPLOYEE WAS OPERATING A 10 SC SHUTTLE CAR WHEN HIS ARM WAS CAUGHT BETWEEN THE COAL RIB AND THE SHUTTLE CAR. HE WAS TREATED BY EMT AND TRANSPORTED TO LOGAN GENERAL. HE WAS DIAGNOSED WITH A DIS LOCATED LEFT ELBOW AND TISSUE DAMAGE. RECOVERY TIME IS UNKNOWN.

EE WAS BLOCKING UP THE PAN OF A MINER WHILE LIFTING IT WITH A SCOOP. THE PAN SLIPPED OFF THE SCOOP BLADE AND LANDED ON THE BLOCKING. A HALF-HEADER FLEW UP AND HIT EE IN THE MOUTH ON IMPACT. EE FINISHED HIS SHIFT & DROVE HIMSELF TO THE HOSPITAL & RECEIVED 8 STITCHES IN HIS UPPER LIP.

MEMBER WAS RIDING ON THE INBY END OF THE MANTRIP WHILE COMING OFF THE SECTION. THE BUS PROCEEDED THROUGH THE FIRST SET OF AIRLOCK DOORS. THEY THEN OPENED THE NEXT SET OF DOORS & PROCEEDED THRO UGH. WHILE THE MANTRIP WAS GOING THROUGH, THE DOORS GRADUALLY CLOSED & CAUGHT THE INBY END CANOPY POST CAUSING THE DOOR TO BREAK DOWN. THE CANOPY LANDED ON INJURED'S RIGHT ARM AND SHOULDER.

EE WAS IN THE LOADER & HE LET THE BUCKET DOWN TOO FAST & HARD. WHEN IT HIT THE GROUND IT JOLTED HIS BACK.

EE & CO-WORKER WERE RECOVERING 60 LB RAIL 1 BREAK INBY OLD #5 HEAD, WHEN THE ACCIDENT OCCURRED. CO-WORKER WAS USING A FORKLIFT TO PULL A 60 LB CURVED RAIL INTO A CROSSCUT. THE RAIL STRUCK THE RIB CAUSING IT TO SWING & STRIKE EE IN RT FOOT.

EE WAS RIDING IN THE FRONT OF A 5 MAN MANTRIP TRAVELING OUTBY @ BRK #7 ON THE NORTH BOXCUT AT THE SAME TIME OF THE INJURY. WHEN THE TRIP STARTED DOWN INTO THE DIP, HE COULD SEE THE MANTRIPS WE RE GOING TO HIT, SO HE JUMPED FROM THE MANTRIP. HE ROLLED TO THE BOTTOM OF THE HILL, RESULTING IN AN INJURED RIGHT KNEE.

EE WAS OPERATING A 5 MAN MANTRIP @ BRK #7 ON THE NORTH BOXCUT TRACK, TRAVELING OUTBY AT THE TIME OF THE INJURY. MANTRIP STARTED DOWN THE BIG DIP & HIT ANOTHER 9 MAN MANTRIP HEAD-ON @ THE BOTTO M. EE WAS THROWN OUT OF THE BUS ONTO THE MINE FLOOR. HE DID NOT HAVE CLEARANCE FROM DISPATCHER.

EE WAS RIDING IN THE BACK OF A 5 MAN- MANTRIP TRAVELING OUTBY @ BRK #7 ON THE NORTH BOXCUT TRACK AT THE TIME OF INJURY. MANTRIP STARTED DOWN THE BIG DIP & HIT ANOTHER 9 MAN- MANTRIP HEAD-ON @ THE BOTTOM. 5 MAN- MANTRIP DID NOT HAVE CLEARANCE FROM THE DISPATCHER.

EE, SHUTTLE CAR OPERATOR WAS LOADING SUPPLIES ONTO THE LEFT ROOF BOLTER THAT WAS SITTING IN THE #3 ENTRY. THE #1 SCOOP THE HE WAS UNLOADING SUPPLIES FROM WAS SITTING BETWEEN #3 AND #4 ENTRIES WITH THE SCOOP BUCKET ON THE MINE FLOOR. THE #2 SCOOP, OPERATED BY SECTION FOREMAN WAS CLEARING THE RIBS IN THE CROSSCUT AND BACKED INTO THE #1 SCOOP, BUCKET STRUCK EE IN RIGHT ANKLE.

EMPLOYEE WAS PUTTING A FORK LIFT ON THE BAR OF THE FORKLIFT AT THE TIME OF THE ACCIDENT. THE FORK DIDN'T CATCH WHEN HE DROPPED IT ON THE BAR AND HIS LEFT HAND WAS SUPPORTING THE FORK. HIS HAND GOT CAUGHT BETWEEN THAT FORK AND THE OTHER FORK CAUSING THE INJURY TO HIS HAND.

EMPLOYEE WAS OPERATING A BROOKVILLE 9 PERSON MANTRIP WITH HIS RIGHT KNEE PROTRUDING FROM THE OPERATOR'S COMPARTMENT. HE STRUCK A TIMBER THAT WAS INSTALLED BETWEEN BELT AND TRACK FOR ROOF CONTR OL PURPOSES AT BREAK #48 ON MAINLINE. HE WAS TAKEN TO LOGAN GENERAL HOSPITAL WHERE HE WAS DIAGNOSED WITH LIGAMENT DAMAGE.

EE WAS A PASSENGER IN THE FAIRCHILD 35C SCOOP WHEN HIS LEFT LEG WAS CAUGHT IN PINCH POINT RESULTING IN A CONTUSION TO LEFT TIBIA. THIS ACCIDENT OCCURED OUTBY THE #1 SECTION ON THE #4 ENTRY HE WAS TRANSPORTED TO LOGAN WHERE XRAYS WERE NEGATIVE RECOVERY TIME UNK

EE WAS IN PROCESS OF CLEANING #2 LEFT BRAKE WITHFAIRCHILD MODEL 35 SCOOP MEMBER STATED THAT HE HIT ON OFF SET IN THE BOTTOM & AT THAT TIME THE RIGHT CORNER OF THE #1 HEADING CAME IN THE CANOPY STRIKING HIM ON THE NECK & RIGHT SHOULDER AFTER GOING TO LOGAN GENL HOSP HE WAS DIAGNOSED WITH ACONTUSSION TO THE NECK & RIGHT SHOULDER
