Employee moving conveyor belt with pallet truck. He felt pull in right shoulder. On 12/5/13 diagnosed with strain. Employee took wait and see approach. After MRI on 02/13/2013 employee diagnos ed partial tear of rotor cuff. Physical therapy prescribed. Employee had 3rd pt on 03/04/2013. Accident now reportable.

Employee was driving a refuse truck, on loan from R&L development. This truck rides rough. The rough ride and rough conditions on refuse pile combined to give employee a bouncy ride. Employee drove the truck all shift. He completed his shift and left. He woke the next am to lower back pain that radiated down his legs making walking hard.

GSI Geo-synthetics (sub contractor) EE was traveling across jobsite on ATV. EE attempted to cross small ditch, when ATV flipped over backwards. The ATV was loaded with sandbags on the rear rack. This offset the balance of the machine, contributing to the turn over.

Employee was operating an A40D Volvo dump truck. In the course of his normal duties of traveling across the refuse dump area, he ran over several rough frozen places. He then complained of a sore neck. He has received three chiropractic treatments as of today. He is working with no restrictions.
