Employee was backing a personnel carrier up when his hard hat caught on a roof bolt thus pushing his hard hat down onto forehead causing a laceration. Employee did not miss any work, however he did receive eight (8) sutures therefore making this report a medical.

He went to back Personnel Carrier up into a nearby x-cut to get out of the way of an incoming Scoop and while doing so the employee jammed his head against the roof. Employee did not miss any work, however he did seek medical treatment making this a reportable.

Employee was riding on a personnel carrier directly behind the operators compartment when apparently his cap lamp cord caught a roof bolt thus pulling his hard hat off his head. As he went to duck down his left shoulder struck the roof and rolled his body back over the fender of the personnel carrier. Employee did not miss any work however he had been placed on restricted duty.
