EE STATED HE WAS PLASTERING A STOPPING ON THE INTAKE SIDE OF THE #5 X-CUT. EE WAS BETWEEN STOPPING & LOOSE CINDER BLOCKS. SCOOP OPERATOR PULLED UP TO PICK UP THE EXTRA LOOSE CINDER BLOCKS. SCO OP PULLED UP ON A ROCK & COULD NOT GET THE SCOOPSTOPPED BEFORE RUNNING INTO THE LOOSE BLOCKS. THIS CAUSED THE BLOCKS TO PUSH FORWARD INTO THE EE, RESULTING IN THE INJURY.

THE EE STATED WHILE RIDING A MAN BUS UNDERGROUND HE LOWER HIS HEAD TO AVOID HITTING A HEADER BOARD IN RESULT HITTING HIS HEAD ON THE MANBUS FRAME . THIS CAUSED A CUT ABOVE THE RIGHT EYEBROW.
