HI S BODY WAS OUT OF POSITION BY LOOKING OVER LEFT SHOULDER, WORKING CONTROLS TO HIS RIGHT, AND STOPPING MOTION FORWARD.

EMP WAS OPERATING A 190 TON HAUL TRUCK, TRANSPORTING A LOAD OF COAL TO THE BOTTOM OF ONE WEST PIT. OPERATOR LOST DYNAMIC BRAKE CONTROL. CAME DOWN A RAMP AND RAN INTO A PILE OF DIRT. CUT HER CH IN ON STEERING WHEEL, REQUIRED 6 STITCHES.
OEPRATOR WAS RUNNING A SCRAPER ON AN APPROX 120' WIDE HAUL ROAD. THERE WAS ANOTHER SCRAPER APPROACHING FROM THE OPPOSITE DIRECTION. A THIRD MACHINE WA STATIONARY BUT WA SOFF THE HAUL ROAD. T HE OEPRATOR THOUGHT THE STATIONARY MACHINE MIGHT PULL OUT IN FRONT OF HIM SO HE IMMEDIATELY STOPPED. HE HITT HIS HEAD ON THE STEERING WHEEL. BOTH SCRAPER OPERATORS STATED THAT THE STATIONARY
THE DRIVER BACKED HIS TRUCK AGAINST A DIRT PILE AT THE DUMP. HE BEGAN RAISING HIS TRUCK BOX BUT DID NOT PUT IT ALL THE WAY UP. AS HE PULLED AHEAD, THE BOX "JACKED" UP DUE TO THE DIRT FALLING O UT. IT THEN SLAMMED BACK DOWN ON THE TRUCK FRAME. THE DRIVER WAS JARRED AND HIT HIS HEAD ON THE STEERING WHEEL.
EMPLOYEE WAS RETURNIGN TO THE SHOVEL AFTER DINNER BREAK, APPARENTLY PAST OUT. OPERATOR DROVE APPROX 300' STRUCK A 6' H 16' W BERM. EMPLOYEE PROCEEDED THROUGHT EH BERM AND DROVE AN ADDITIONAL 300' ACROSS A COAL BENCH BEFORE GOING OVER TH E CREAT OF THE COAL FACE.
ELECTRICIAN WAS CLEANING COMMUTATOR ON WHEEL MOTOR. THE STONE CAUGHT ON SOME SLAG AND HIS HAND WAS PULLED INTO A BRUSH HOLDER BRACKET.
EMPLOYEE HAD RECEIVED TREATMENT FOR NECK PAIN IN AUGUST AND NOVEMBER OF 1992. HE WAS TREATED FOR A CERVICAL STRAIN AS A RESULT OF TIPPING HIS HAUL TRUCK OVER ONTO ITS SIDE ON 12-30-92. CHIROPR ACTOR STATED THIS HERNIATED/RUPTURED DISC SURGERY ON 4-8-93 WAS THE RESULT OF ACCUMULATIVE DISC PROBLEM. EMPLOYEE OFF FOR APPROX. 16 WEEKS.

INJURED STARTED EMPLOYMENT OPERATING SCRAPER HAULING TOPSOIL ON 03-06-91.HE WAS MOVING AT APPROXIMATELY 6MPH,GETTING READY TO SPREAD HIS LOAD.HE TURNED HIS HEAD AND BODY TO THE RIGHT AND FELT A SHARP PAIN IN HIS LOWER BACK.HE FINISHED SPREADING HIS LOAD OF MATERIAL AND DROVE THE SCRAPER OFF TO ONE SIDE OF THE FILL AREA AND WAITED FOR HELP TO ARRIVE.HE THOUGHT HIS BACK HAD POPPED OU
DOZER WAS CLEANING OUT TRUCK BOX WHEN IT STRUCK HAUL TRUCK. TRUCK OPERATOR WAS JOSTLED AROUND IN CAB. NEXT DAY IT WAS REPORTED TO SUPERVISORS FOR INVESTIGATION. TRUCK DRIVER THEN REPORTED SORE
LOWERIG TRUCK BOX ON DUMP BOX WAS LOWERING IT STOPPED ABRUPTLY JARRING TRUCK AND DRIVER NOF DEFECTS WERE FOUND IN TRUCKS HYDRAULIC SYSTEM AND NONE HAD BEEN REP. TO DAT E ON THIS TRUCK.
TRUCK DRIVER DROVE THROUGH SMALL DIP IN ROAD AND SEAT BOTTOMED OUT CAUSING PAIN TO EMPL LOWER BACK.
TRUCK BEING LOA8ED WITH 2 ND BUCKET OF MATERIAL WHEN A ROCK WHICH WAS WEDGED IN BUCKET CAME FREE. THE ROCK STRUCK THE LEFT REAR INSIDE OF THE BOX AND BOUNCED TO THE OPPOSITE SIDE. THIS CAUSED THE TRUCK TO R2CK AND THE EMPLOYEE INCURRED A NECK STRAIN.
THE HAUL TRUCK 4AS GETTING LOADED FROM SHOVEL. THE HAUL TRUCK OPERATOR STATED THAT THE SHOVEL BUCKET DOOR STRUCK THE CANOPY OF THE TRUCK. THE SHOVEL OPERATOR AND OILER STATED THAT THEY DID NOT SEE OR FEEL TH9 SHOVEL BUCKET STRIKE THE HAUL TRUCK.
WHILE ATTEMPTIN1 TO INSTALL THE LEFT FRONT FR AMEWORK ON A HAUL TRUCK THE FRAMEWORK TWISTED. IT WAS SUPPORTED BY A 2-TON COME-ALONG AND A FORKLIFT. AS THE FRAMEWORK NEARED THE DESIRED LOCATION IT CAME FREE FR2M THE FORKLIFT PERMITTING IT TO TURN ON THE COME-ALONG CABLE. THE FRAMEWORK STRUCK THE SUPERVISOR IN THE RIGHT THIGH
BACK OF SEAT ON 186.12 HAD BEEN BROKEN PRIOR TO SHIFT & NOT REPORTED ON OPERATORS REPORTS I WAS TAPED TOGETHER WHEN DUMPED THE BED JARRED AS THE DUMP CYLINDERS STOPPED WHICH BROKE THE SEAT LO OSE
A GUARD WAS BUILT FOR THE HEATER FAN ON THE FUEL TRUCK AT THE BEGINNING OF THE SHIFT. BEFORE IT WAS INSTALLED IT WAS DISCOVERED THAT A FAN BLADE WAS BROKEN UNDER THE EXISTING GUARD AND ONE FAN WASN\T WORKING. AFTER THE FAN WAS CHANGED THE EMPLOYEE WAS CHECKING IT. IN DOING THE CHECK HE GOT HIS LITTLE FINGER RIGHT HAND INTO THE FAN BLADE.
EMPLOYEE WAS ASSISTING IN CHANGING A RETURN BELT ROLLER. VICTIM WAS HOLDING A ROPE ATTACHED TO THE ROLLER, THE ROLLER APPARENTLY MOVED SUDDENLY CAUSING THE VICTIM TO BE THROWN AGAINST SOMETHIN G. VICTIM RECEIVED CUT TO LOWER LIP REQUIRING 10 SUTURES.
HAULING TOPSOIL FROM T-33 TO T-48 RUNNING LEFT HAND TRAFFIC FOR CROSS TRAFFIC SAFETY. OPERATOR IN UNIT NO. 3004 FORGOT AND TURNED CORNER IN WRONG LAND HITTING UNIT NO. 3001 HEAD ON.
