RAN OVERHOLE IN HAULAGE ROAD CAUSING SEAT IN BATTERY RAMCAR TO BOTTOM OUT JAMMING BACK. NO SCHEDULED TO WORK UNTIL 12-30-96. BACK WAS STILL HURTING 1-2-97 SO HE WENT TO DR AND FILED ACCIDENT R EPORT. DR PLACED HIM ON RESTRICTED DUTY BEGINING1-3-97.
ROCKDUST TRAILER HE WAS PULLING CAME UNCOUPLED. HYDRAULIC HOSE FROM SCOOP TO CAR STRETCHED THEN BROKE. THE COUPLER STRUCK HIM ON CHIN CAUSING LACERATION THAT REQUIRED STITCHES.
WHILE DRIVING BATTERY RAMCAR, EE RAN OVER A POT HOLE IN HAUL ROAD. THIS CAUSED EE TO BOUNCE OFF SEAT STRAINING HER BACK. TREATED & RELEASED ON THIS DATE BUT CONT TO HAVE PAIN AND REVISITED DR ON 11/11/96. BED REST FOR 2 DAYS THEN RESTSRICTED DUTY.
HE WAS OPERATING SCRAPER HAULING REFUSE, HAULROADS AHD BEEN WATERED CAUSING SCRAPER TO SLIDE. WHEN SCRAPER SLID, EE STRUCK DOOR HANDLE. THE IMPACT CAUSED FRACTURE TO RIBS.
USING CHAIN TIED TO SCOOP AND SLATE BAR TO DISLODGE A LARGE PIECE OF ROCK FROM MINER CONVEYOR. CHAIN SLIPPED AS HE WAS PRYING WITH SLATE BAR, CAUSING BAR TO FLIP UP STRIKING HIM ON RIGHT SIDE OF FACE, FRACTURING HIS JAW.
EE WAS STANDING 2' INGBY PULL THRU CURTAIN IN #4 ENTRY OF 5L2MMU STATION 4+35 TALKING TO PRODUCTION COORDINATOR. A RAM CAR PULLED THRU CURTAIN AND BUMPED BOTH MEN. IN AN ATTEMPT TO GET OUT OF WAS, EE STRAINED HIS RIGHT KNEE AND WAS PLACED ON RESTRICTED DUTY.
DRIVING DIESEL TRACTOR, RAN OVER SULFUR BELL IN TRAVEL WAY. CAUSING HIM TO BOUNCE INTO CAB OF TRACTOR. HAD PAIN IN NECK AND LOWER LEFT SIDE OF BACK. SAW DR ON 11-10-95 WHO PRESCRIBED MEDICATIO N AND PLACED HIM ON RESTRICTED DUTY FOR ONE WEEK
ON SEPT 18,1995 WHILE DRIVING A BATTERY RAM CAR, HE RAN OVER A ROCK IN HAUL ROAD CAUSING HIM TO BOUNCE OFF SEAT AND STRIKE HEAD ON CANOPY HAS HAD PAIN IN NECK SINCE THEN WENT TO DR ON FEB 16,1 996. DIAGNOSED WITH CERVICAL STRAIN AND WAS PLACED ON RESTRICTED DUTY.
HE WAS OPERATING BATTTERY SCOOP, RAN OVER A ROCK IN TRAVEL WAY AND WHEN SCOOP BOUNCED HE PULLED MUSCLE IN BACK. WENT TO DR ON 6-26-95 AND WAS PLACED ON RESTRICTED DUTY.
WHILE OPERATING A BATTERY RAM CAR HE FELT A PAIN IN NECK & RT SHOULDER PAIN GOT PROGRESSIVELY WORSE & HE WENT TO DOCTOR 06-13-95 & WAS DIAGNOSED WITH STRAINED RIGHT TRAPERIUS MUSCLE & PLACED O N RESTRICTED DUTY HE BELIEVES STRAIN WAS CAUSED BY TWISTING NECK FOR VISIBILITY WHILE DRIVING RAM CAR
HE WAS PASSENGER ON A BATTERY POWERED PERSONNEL CARRIER. IT WAS BEING BACKED UP SO A DROPPED TOOL COULD BE RECOVERED A SECOND PERSONEL CARRIER WAS TRAVELING INBY AND THE TWO COLLIDED THE IMPAC T CAUSED A STRAIN AND CONTUSION TO HIS BACK. HE SAW A DR ON 4-24- AND 4-25 WAS PLACED ON RESTRICTED DUTY.
WHILE OPERATING SCRAPER, HE WAS BOUNCED IN CAB FROM RUNNING OVER HOLE IN HAUL ROAD. SUFFERED LACERATION TO FOREHEAD THAT REQUIRED STITCHES. WAS RELEASED TO RETURN TO WORK.
HE SHUT LID ON BATTERY SCOOP AS IT WAS MOVING. HIS LEFT HAND WAS CAUGHT ON BATTERY FRAME (GLOVE) AND LID FELL CATCHING HIS LEFT THUMB BETWEEN LID AND BATTERY CASE. THE IMPACT CRASHED AND SEVER ED HIS LEFT THUMB AT FIRST JOINT.
HE WAS ATTEMPTING TO DISLODGE A PIECE OF WIRE ROPE FROM UNDER BELTLING. AN END OF THE ROPE FLIPPED UP AND WAS PULLED INTO MOVING BELT BETWEEN BELT AND TOP ROLLER, TAKING HIS RIGHT HAND ALONG. THE MOVING BELT CAUSED A FRICTION BURN TO TOP OF HAND. ON 7-5-95 HE HAD PLASTIC SURGERY TO CORRECT SCARRING FROM THE BURN.
THIS ACCIDENT OCCURRED INITIALLY APRIL 10,1991. HE HAS HAD BACK TROUBLE OFF AND ON SINCE THEN. WENT TO DR ON 11-15-93 AND WAS PLACED O RESTRICTED DUTY UNTIL FURTHER TESTS ARE RUN AT TIME OF A CCDIENT HE WAS OPERATIONG A SCOOP AND WAS BOUNCED AROUND IN CAB DUE TO ROUGH ROADS.
HE WAS OPERATING AN S&S SCOOP WHEN HE RAN OVER A 6"X6" CRIB THAT WAS IN ROADWAY. THIS CAUSED HIM TO BOUNCE OFF OF SEAT AND STRIKE HEAD INTO CANOPY. HE SOUGHT MEDICAL ATTENTIO FOLLOWING DAY 1 0-22-93 AND WAS TOLD TO STAY OFF FOR 10 DAYS PENDING TEST RESULTS.
NECKHAD BEEN BOTHERING HIM FOR COUPLE MONTHS RELATES CAUSE TO HITTING BUMPS IN HAUL ROAD WHILE OPERATING SCOOP ON JUNE 6/93 WHILE BOLTING PAIN GOT WORSE WHILE TURNING HEAD TO LOOK AT TOP WENT TO DOC & WAS PUT ON RESTRICTED DUTY FOR THREE WEEKS
RAM CAR CAUGHT WATER LINE & STRETCHED ACROSS ENTRY ENTANGLING HIS LEG, AND DRAGGING HIM ACROSS ENTRY. THIS HAPPENED JAN 8, 93. EE HAS HAD TROUBLE WITH KNEE SINCE THEN HAD SURGERY ON 7-793 TO C ORRECT AND THEN BECAME A LOST TIME ACCIDENT.
