SHUTTLE CAR WAS STUCK UP AND A SCOOP WITH A WINCH WAS BEING USED TO PULL SHUTTLE CAR AROUND. FLOOR UNDERNEATH SHUTTLE CAR GAVE WAY AND CAR SLID SUDDENLY, STRIKING EMPS LEFT LEG.

EMPLOYEE WAS OPERATING A SHUTTLE CAR. HE HAD HIS LEFT LEG PARTIALLY OUTSIDE THE CANOPY. EMPLOYEE STRUCK THE MINE RIB WITH THE SHUTTLE CAR PINCHING HIS KNEE BETWEEN THE RIB AND THE SHUTTLE CAR.
