SCOOP OPERATOR WAS CHEANING ROAD LEADING TO THE TAIL PIECE IN THE #4 ENTRY. MAKING A TURN FROM #4 TO THE #3 ENTRY, THE OPERATOR DID NOT ALLOW ENOUGH ROOM FOR THIS TURN. HANGING CLOSE TO THE RI B, THE OPERATOR CAUGHT HIS LEFT EAR & LEFT SIDE OF THE HEAD ON SOME LOW HANGING COAL.

THE INJURED PERSON WAS HANGING CURTAIN IN FACE NEAR THE REAR OF MINER. HAVING HIS BACK TOWARDS THE MINER, SHUTTLE CAR OPERATOR COULD NOT SEE WORKER, AND SLIGHTLY CAUGHT THE WORKER IN HIS BACK WITH THE END OF THE SHUTTLE CAR.
