EMPLOYEE STATED HE WAS RIDING A SCOOP BUCKET WHEN HIS RIGHT INDEX FINGER WAS CAUGHT BEHIND THE SCOOP RAM. EMPLOYEE WENT TO THE DR. WHO REMOVED THE LAST JOINT OF THE FINGER.

EE STATED THAT HE WAS SHOVELING THE OUTSIDE BELTDRIVE. A SPILL HAD OCCURED AND HIS HARD HAT FELLOFF DUE TO THE BENDING POSITION HE WAS IN. BEFORE REPLACING HIS HAT, A SMALL PIECE OF ROCK CAME OFF THE BELT AND STRUCK HIM IN THE TOP OF THE HEAD. THE LACERATION REQUIRED 3 STITCHES.
