Employee was adjusting forks on Lo-trac. When he slid the fork over, the fork fell catching his finger between the fork and flat car resulting in a fracture.

While operating the shuttle car near #3 intersection near spad 3106, member struck his head on canopy when he ran over something. There were no witnesses, member was taking by EMTs on back board to local hospital where evaluation showed contusion on lower spine.

Employee was making the fireboss run through the mine, while traveling in the mantrip, he hit a rock that had fallen on the No. 3 Belt. The rock turned into the operator's compartment striking employee in the right side.

INJURED WAS SITTING IN THE OPERATOR'S SEAT OF A BATTERY POWERED MAC8, STEERING IT WHILE BEING PUSHED BY A SCOOP TO THE BATTERY CHANGING STATION. THE MAC8 BATTERIES WERE DEAD. WHILE IN ROUTE, T HE SCOOP PUSHED THE MAC8 OFF THE GROUND APPROX. 12" JARRING THE INJURED CAUSING LOWER BACK PAIN.

INJURED WAS OPERATING A 21 SC SHUTTLE CAR ON THE #2 SECTION MMU 003-0 WHILE TRAVELING 1A THE #5 ENTRY 40' INBY THE INTERSECTION, A RIB OF APPROX. SIZE 29"L X 36"W X 1" TO 6" THICK FELL INTO TH E OPERATOR'S COMPARTMENT, STRIKING HIM ON THE RIGHT LEG, RESULTING IN POSSIBLE KNEE DAMAGE.

EE WAS CLEANING THE SANDER ON RAIL EQUIPMENT WITH HIS FINGERS.A FELLOW EE ACTIVATED THE SANDER TO CHECK ITS OPERATION, NOT KNOWING THAT THE INJURED EE'S FINGERS WERE IN IT. THE GATE OF THE SAN DER DISCHARGE CLOSED ON HIS RIGHT INDEX FINGER, SEVERING THE FINGER TIP.

EE WAS OPERATING A JITNEY RAILRUNNER WHEN HE FAILED TO SLOW DOWN AT THE OLD 4 SPUR SWITCH WHICH WAS BEING USED A DERAIL. HE RECKED AND CAUSED IMPACK TO HIS TAILBONE. HE COMPLETED HIS SHIFT AND WENT TO DR ON THE 18TH.
